Response to 85-12-01' Independent Review of Design Change Calculations _

Corrective -Steps Which Itave Been Taken and the Results Achieved i

a.

The NRC Inspector reviewed 53 Engineering Mechanics Division (EMD) calculations and identified three calculations which had not been signed off by an independent reviewer. A subsequent Engineering Assurance surveillance reviewed 471 additional calculations and identified two calculations which had not been

signed off by an independent reviewer. Thus, five calculations were identified out of a total sample of 534 calculations.

Only one of the five calculations identified had been processed after implementation of the preventative action asscciated with Engineering Assurance audit A0-12210-195 conducted on October 12, i

1984, which reported a similar problem.

7he five calculations identified have been reviewed and approved.

A sample review of the structural calculation books examined by the NRC indicated that approximately six percent of all calcu-

lation pages did not exhibit an "I" next to the reviewer's signature or the independent reviewer's signature.

A 100 percent review of all SEG structural calculations was subsequently performed.

Where any deficiency was noted, the independent review was appropriately documented and reflected in calculation files.

'2.- Response to 85-12-02 Control of Temporary Alterations Corrective Steps' Which llave Been Taken and the Results Achieved Corrective action was taken immediately and in the presence of the NRC Inspector to bring temporary alteration 85EGS-009 into compliance with River Bend Startup Manual Test Instruction No. 17.

The temporary alteration log was then audited by the GSU Project Quality Test Group (PQTG). The audit was completed by March 31, 1985,' and the minor discrepancies identified by the PQTG have been corrected to ensure compliance with Test Instruction No. 17.

Corrective Steps Which Will Be Taken To Avoid Further Violations GSU's Startup and Test Group has assigned dedicated personnel to assure that procedural compliance is maintained. These personnel will control entries to the temporary alteration log and verify correct installation and/or removal of temporary alterations.
