FAILURE OF DIESEL GENERATOR (UPDATE)

A preliminary evaluation by the licensee and Region III (Chicago) had determined that the IB diesel generator failure was caused by an improperly torqued bolt in the articulated connecting rod. A review of quality assurance records, interviews with licensee personnel, and physical examination of the components indicate that the failure resulted from the improper torquing of the bolt during a piston replacement which occurred during the current outage. The failure occurred during post-maintenance testing. Metallurgical examination of the failed bolt showed the failure mechanism to be fatigue.

The licensee has examined two of the four other diesel generators on site. Several bolts were found that had suspect torque valves; these bolts have been replaced and torqued to the proper value. Based on the current review, it does not appear that the failure has generic implications. The licensee plans to examine the remL:ining diesel generators.

Repair activities on the diesel generator are continuing. Region III (Chicago) is monitoring the repair work.

The State of Illinois will be informed.
