NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50 295; 50 304 Zion Nuclear Generating Station License Nos. DPR 39; DPR-48 During an NRC inspection conducted on May 30 through July 18,1997, three violations of NRC requirements were identified in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600, the violations are listed below:

1. 10 CFR Part 50, Appendix B, Criteria V, " Instructions, Procedures, and Drawings,"

requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and be i accomplished in accordance with these instructions, procedures, or drawings.

Operating Special Procedure (OSP)97-012, "2A Diesel Generator Cylinder Liner Test," Revision 1, Section 5.58, requires that the 3L Klone valve be closed.

Contrary to the above, on May 10,1997, during performance of OSP 97 012, Section 5.58, the licensee failed to close the 3L Kiene valve.

2. 10 CFR Part 50, Appendix B, Criteria V, " Instructions, Procedures, and Drawings,"

requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and be accomplished in accordance with these instructions, procedures, or drawings.

Contrary to the above, on June 26,1997, Technical Staff Surveillance Procedure 008 97, " Diesel Generator 0 Governor Test Fast and Slow Starting Modes Construction Test," was not appropriate to the circumstances in that it did not provide appropriate guidance for setting the droop potentiometer prior to loading the 0 emergency diesel generator (EDG); consequently, the govemor droop potentiometer was set at zero instead of mid scale. As a result, when the O EDG was paralleled to the grid, operator action was required to prevent potential overloading of the EDG.

This is a Severity Level IV violation (Supplement IV). (50 295/97016-05; 50-304/97016-05)

3. 10 CFR Part 50, Appendix B, Criteria X, " Inspection," requires, in part, that a program for inspection of activities affecting quality be established and executed by or for the organization performing the activity to verify conformance with documented instructions, procedures, and drawings for accomplishing the activity.

Notice of Violation 2-Work Package No. 950069039-01, "O D/G IDiesel Generator] Governor Outage - Electrical," Step 38, specifies, in part, install connector and terminate cable 59059 at OSZ-DG86 in accordance with Document #101, " Soldering Procedure for Electrical Components."

Document #101, " Soldering Procedure for Electrical Components," requires, in part, that the original wire strand lay be maintained, that stranded conductors be tinned resulting in a thin coat of solder on all strands bonding them together, and that once the conductors are positioned, the excess conductor ends be trimmed.

Contrary to the above, on June 2,1997, the quality control inspection conducted for the installation of the amphenol connector and termination of cable 39059 was not adequate to verify conformance with the work instructions, in that, a loose conductor strand at the amphenol connector was not identified. This strand subsequently caused a ground which resulted in a protective trip of the O EDG on June 25,1997.

} the Regional Administrator, Region ill, and a copy to the NRC Resident inspector at the t

facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addtesses the recuired response. If an adequate reply is not received within the t;me specified in the Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
