1 NOTICE OF VIOLAT!ON Commonwealth Edison Company Docket Nos. 50-295; 50-304 Zion Generating Station, Units 1 and 2 License Nos. DPR-39; DPR-48 As a result of an inspection conducted on October 27-31,1997, a violation of NRC require-ments was identified. In accordance wi'.h the " General Statement of Po"cy and Procedure for NRC Enforcement Actions," NUREG '.600, the violation is listed below:
Technical Specification 6.2.2.8 requires areas accessible to personnel with radiation levels greater than 1000 millirem per hour (millirem /hr) at 30 cm from the radiation source or from any surface which the radiation penetrates shall have locked doors to prevent unauthorized entry.
Procedure ZAP 610-2, "High Radiation Area Access Control," revision 5, requires that entrances to act essible high radiation areas, with radiation levels greater than 1000 millirem /hr, be locked or be controlled by a key custodian who has direct oversight of and positive control over each personnel entry into the area.
Contrary to the above, on October 17-18,1997, the entrance to the truck bay of the radioactive waste area, a high radiation area with radiation levels greater than 1000 millirenvhr, was not locked and personnel did not have direct oversight of and positive control over each entry into the area. (50-295/97026-01 and 50-304/97026-01)
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington D.C. 20555 with a copy to the Regional Adrainistrator, U.S. Nuclear Regulatory Commission, Region til,801 Warrenville Road, Lisle, Illinois 60532, and a copy to the NRC Resident Inspector at the facility which is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).1his reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may referenca or include previous docketed correspondence, if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in tnis Notice, an order or a demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
Because your response will be placed in the NHC Public Document room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redac ion. If personal privacy or proprietary information is necessary to provide an acceptable response, then please provide a bracketed copy of your response that identifies the information that should be protected and a redacted copy of your response that deletes such information.
