The purpose of this letter is to provide the NRC with a status of Waterford 3's actions to address a recent concern involving the discovery of suspect / missing fasteners on safety-related motor operated valves (MOVs), as requested by NRC Region IV personnel during a conference call on October 2,1996. On October 1,1996, Waterford 3 personnel initiated a walkdown of safety-related MOVs to inspect critical components for the correct material fasteners. This walkdown was a follow-up action from Condition Report (CR) 96-1455, which identified an apparent incorrect material for the limit switch cover mounting cap screws and washers for four safety-related MOVs on September 19,1996. The material of the cap screws was originally questioned by a visiting NRC inspector during a plant walkdown. The limit switch cover screws were determined to be non-critical components for the valves and did not present an operability concern. Waterford 3 engineering personnel pursued the question further and determined that an inspection of critical MOV fasteners was warranted to determine if a generic issue of fastener control exists. The walkdown initiated on October 1,1996 revealed suspect or missing fasteners on three safety-related MOVs. The following immediate actions were taken:

. CR 96-1528 was initiated to implement the Waterford 3 corrective action process.

. Parallel efforts were taken to continue the MOV inspections, identify replacement -

parts for the suspect / missing fasteners, and evaluate the effect of the discovered ,

discrepancies on operability of the valves. It should be noted that, based on i preliminary review, none of the identified discrepancies affected operability of the ,

valves.  ;

. A Root Cause Analysis Team has been formed for CR-96-1528. This cross-discipline team is responsible for determining the root cause and necessary ,

corrective actions for the specific issue of fastener controls with respect to MOVs -

as well as determining any generic implications to ensure the issue is fully bounded and resolved. ,

. The details of this issue were discussed with each of the Maintenance disciplines and Construction personnel on October 2,1996 to heighten awareness in the area of fastener controls. Waterford 3 Maintenance personnel currently at l Arkansas Nuclear One (ANO) for outage support were also contacted and briefed i on the issue. 1 e Licensing and plant management personnel at the other Entergy nuclear sites, namely ANO, Grand Gulf, and River Bend, were notified of the MOV fastener discrepancies identified at Waterford 3.

On October 2,1996, a conference call was held between Waterford 3 personnel and NRC Region IV personnel to discuss the status of the identified discrepancies and associated generic concerns. During the call, the NRC expressed several concerns with regard to the issue of fastener control at Waterford 3. A status of the immediate actions taken and current planned actions was discussed during the conference call.

The NRC requested that Waterford 3 document the action plan to address this issue in a letter to the staff. Attachment 1 provides a summary of the current action plan requested, which includes actions that have been completed as well as those in progress and planned. It is important to note that the results of certain actions listed may warrant further actions to be taken as deemed necessary until the issue is fully bounded and resolved.

As a result, Waterford 3 is taking an aggressive and comprehensive approach to addressing the concerns as outlined in the attached action plan. These actions, along with any additional actions deemed necessary through further investigation, will ensure the question of fastener / parts control at Waterford 3 is bounded and resolved completely. ATTACHMENT 1 Current Action Plan For CR-96-1528 The following is a current list of corrective actions to address the condition identified in CR-96-1528 related to suspect / missing fasteners on safety-related MOVs. A cross-discipline Root Cause Analysis Team has been formed with the purpose of determining the root cause and necessary corrective actions for the specific issue of fastener controls with respect to MOVs as well as determining any generic implications to ensure the issue is fully bounded and resolved.

Specific Actions to Address MOVs A walkdown and externalinspection of all safety-related and Category 1 (GL 89-10) non-safety-related MOVs at Waterford 3 has been completed (78 total). Attachment 2 provides a list of these valves, the type of discrepancies identified and the required actions. A preliminary review has indicated that the operability of these valves were not affected by the identified discrepancies.

It is important to note that the MOV inspection distinguished between carbon steel and stainless steel fasteners. The stainless steel fasteners were considered suspect in that they may not meet the appropriate SAE r,rade material strength requirements. With the exception of valves SI-415 A&B, allidentified stainless steel fasteners were replaced with the appropriate SAE Grade carbon steel fasteners.

The SI-415 A&B fasteners replacements are scheduled to arrive on site on October 7,1996. A number of carbon steel fasteners were also replaced. A sample of the  ;

stainless and carbon steel fasteners which were removed are being analyzed to I determine if they meet the appropriate SAE Grade material strength requirements. l The results of this analysis will help to determine the scope of further sampling of installed fasteners.

Inspection of the internal fasteners for a sampling of MOVs will be performed. A review is being performed to determine the scope, function and appropriate sample size of internal MOV fasteners. This review will be completed by October 10,1996.

Actions to Address and Bound Generic Question of Fastener / Parts Control The details of the this issue were discussed with each of the Maintenance disciplines and Construction personnel on October 2,1996 to heighten awareness in the area of l

fastener controls. Waterford 3 Maintenance personnel currently at Arkansas Nuclear '

One (ANO) for outage support were also contacted and briefed on the issue.

namely ANO, Grand Gulf, and River Bend, were notified of the MOV discrepancies identified at Waterford 3. Waterford 3 will continue to communicate with the other sites in resolving the concerns.

4 The Quality Assurance (QA) Department interviewed Electrical Maintenance, Mechanical Maintenance, I&C Maintenance, Construction, and Planning and Scheduling personnel in an attempt to identify the rwrmal practices used by the craft personnel and the expectations given with respect to the replacement of fasteners.

These interviews were performed anonymously. A brief summary of the interviews is

, included in Attachment 3. These results will be evaluated by the Root Cause Analysis Team to determine any necessary corrective actions with regard to expectations on worker practices.

Per the responses to the interviews discussed above, two components were identified as potentially having incorrect fasteners, namely the mounting screws for valve CC-135A pressure switch, and the motor termination cover bolts on LPSI i Pump B. These components are being promptly investigated.

a A review of the work history for a number of the MOVs listed in Attachment 2 is being 4

performed in an attempt to identify when the discrepancies may have been

introduced. This review may also help to bound the problem from a personnel ,

standpoint. The majority of maintenance performed on MOVs is by the electrical l maintenance discipline. If the review identifies that the discrepancies were l introduced by certain plant personnel, or contractors, other work performed by the individuals will be reviewed for quality.

1 A review of other types of components which fall under the Electrical Maintenance l discipline is being performed to determine if they are susceptible to fastener control problems. It should be noted that reactor trip breakers do not appear susceptible due to the fact that they are sent off-site for rnaintenance and repair.

A 10% sample of. safety-related air operated valves (AOVs) has been selected for external fastenerinspection. This sampling includes 24 safety-related AOVs selected based on Probabilistic Risk Assessment importance. A walkdown of these valves to identify fastener inconsistencies or missing fastenere All be completed by October 4,1996. The results of this walkdown will determine if tne scope of AOVs needs to be expanded, and may help to bound the issue from an equipment / discipline standpoint since most AOV maintenance is performed by the mechanical and l&C disciplines.  !

To broaden the review beyond safety-related valve actuators, an inspection of a  !

sampling of accessible, critical Emergency Diesel Generator fasteners will be performed, with emphasis placed on fasteners that are frequently removed and replaced during routine maintenance (electrical, mechanical, and I&C). The as-found field-installed fasteners will be compared to appropriate configuration-control documentation to ensure the as-found are the correct fasteners. The engine

l manufacturer will also be consulted to ensure correct fasteners are installed. The inspection will be completed by October 18,1996. This action may help to bound the issue from an equipment standpoint.

Condition Report database searches as well as Condition Identification (Cl) and Work Authorization (WA) database searches have been performed to look for past prob; ems involving incorrect / missing fasteners and materials controls problems which may have been identified and documented. The results of these searches are being reviewed for applicability to the current issue and will be included in the efforts of the Root Cause Analysis Team to determine further necessary corrective actions.

These searches may also help in bounding the problem.

It was discovered that some of the suspect / missing fasteners identified did not have warehouse stock numbers cross-referenced to part numt,ers. Typically these parts can be found by cross-referencing part descriptions and specifications. QA will perform a process survey, which will include a review of work packages, to ensure spare parts are being requested and issued in accordance with procedural requirements and that the current procedures ensure parts are being replaced with correct spares. The expected completion date of this process survey is October 31, i 1996. l l

I ATTACHMENT 3 Summary of interviews on Fasterer Work Practices Assessment Scope:

This assessment was conducted at the request of Plant Management to document normal maintenance practices for replacement of safety related fasteners. This was accomplished by face to face interviews with 82 personnel from PME, PMC, PMM, and NOC. Additionally,24 maintenance personnel assigned to ANO were interviewed by telephone. Planners and craft personnel from each maintenance organization and Supervisors, Field Engineers and contract craft personnel from NOC were included in the survey. A total of 106 personnel were interviewed. A questionnaire which was developed by plant staff was written up by OA for each person interviewed.

The four questions asked in the assessment were:

1. If you are performing a job and need a replacement fastener, what is your normal practice for replacing these fasteners on safety related components?
2. Do you inform your supervisor and document what you have done, if you identified a problem with fasteners?
3. What is the direction that you were given related to fastener replacement on safety related components?
4. Do you know of any specific safety related components that have fasteners installed that are not like for like replacements? Have you ever found any and what did you do after you found them?

Summary of Results:

As a result of this assessment it was concluded that plant personnel are cognizant of and adhere to program requirements for the replacement of safety related fasteners. It is a routine acceptable practice to use Q3 fasteners which have been up-graded by Engineering Evaluation. There are indications that in some cases, due to scheduling or a shortage of stock items in the warehouse, personnel have had the perception of being pressured to do whatever is necessary to get the job done. Based on responses to question #4, there are two components that may need to be I evaluated. The mounting screws for valve CC-135A pressure switch, and the motor termination cover bolts on LPSI Pump B were identified as potentially having incorrect i fasteners.
