i APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-311 Salem Unit 2 License No. DPR-75 As a result of. the' inspection ~ conducted on January 27, 1987 February.23, 1987, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2, Appendix C (Enforcement Policy

- 1986), the following violation was identified:

A. Technical Specification Surveillance Requirement 4.6.1.1.a requires, in part, that primary containment integrity shall be demonstrated at least once per 31 days by verifying that all penetrations... required to be closed during accident conditions, are closed by valves, blind flanges or deactivated automatic valves secured in their positions.

Contrary to the above, on January 29, 1987, it was determined that 1 , primary containment integrity was not demonstrated at least once per 31 days since five containment boundary valves (2CS64, 2SJ229, 2SJ293, 2CV309 and 2CV365), associated with a single penetration required to be closed for certain accident conditions, had not been verified closed because the valves were omitted from the implementing proc <tdure.

This is a Severity Level IV violation (Supplement 1) applicable to DRP-75.

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply,. including for each violation: (1) the corrective steps.which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown,

- consideration will be given to extending the response time.
