This will acknowledge and respond to your letter of June 16, 1981 (received June 19,1981) regarding Inspection No. 50-201/81-03 conducted by Mr. P. Clemons on April 22-24, 1981, of License No.

CSF-1 activities. Your inspection indicated one item of non-compliance, which is discussed in tl.e enclosure to this letter.

We believe the corrective action specified in the enclosure will prevent recurrence.

Enclosure - J. P. Duckworth to Nuclear Regulatory Commission, Region 1, July 16, 1981 NFS RESPONSE TO USNRC REGI'ON I LETTER TO NFS JUNE 16, 1981 INSPECTION NO. 50-201/81-03 NRC ITEM A.. Technical Specification 7.1.4 states, " Detailed written procedures, run sheets, letters of authorization and extended work procedures shall be provided for operation of the plant. In addition, procedures for handling abnormal operating conditions and for radiation protection shall be provided." Item 3.4 of Section 3, Special Work Permit (SWP),

of the Health and Safety Manual, developed oursuant to the above states, "all entries into zone 4 areas or high radiation areas require a Special Work Permit (SWP) . . .

Whenever anyone except Health and Safety has to work alone in a zone 4 or high. radiation area, a qualified individual must be assigned to check on the wellbeing of the person in work area. This backup person must sign the SWP on the front beneath Health and Safety Technician."

Contrary to the above, on Februar 13, 1981, Special Work Pennit No.

9721 "(1) Calibrate instrument (y)c Lube valves" was issued to cover operations in a high radiation area. The Special Work Permit required a backup person to check on the wellbeing of the person in the work area, but the backup person failed to sign the Special Work Permit as required. The work was done en the date cited above.

This is a Severity Level VI Violation. (Supplement IV)

NFS RESPONSE The corrective steps which have been taken and which will be taken to avoid further violations are as follows:

1. A notice to all employees was posted reiterating the purpose, procedure and importance of Special Work Permits (SWP's). The notice was posted July 15,-1981.

2. A special audiovisual training program will be produced and provided to employees and documented as a monthly safety training program on the perpose, practices and importance of Special Work Permits. The program

. w 11 be conducted in Augus+ 1981 and will be completed by August 31, 1981.

3. Instruction was given to Health and Safety Specialists to provide the rauiation worker (s) with a Special Work Permit for final signature after the SWP has been duly signed by Supervision, Health and Safety and the backup person when specified. This instruction was provided and documented July 15, 1981.
