Box 800 2244 Walnut Grove Avenue Rosemead, California 91770 Docket Nos. 50-361 and 50-362 Construction Permits No. CPPR-97 and CPPR-98 NOTICE OF VIOLATION Based on the results of NRC inspections conducted on September 18-22, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Construction Permits No. CPPR 97 and CPPR-98 as indicated below. These items are infractions.
A. 10 CFR 50 Appendix B, Criterion X, as implemented by the quality assurance program delineated in Chapter 17 of the PSAR, requires that the program established for inspection of activities affecting quality shall be executed to verify conformance with the documented instructions, procedures, and drawings for accomplishing the activity.
- 1. Bechtel Construction Procedure No. WPP-AC1-103, Rev. 10 states that the minimum concrete cover for reinforcing bars of sizes No. 11 or less shall be 3/4 inch.
Contrary to the above, on September 19, 1978, reinforcing steel bars of size No. 5 for concrete placements 23-883A and 23-884A for Unit 3 containment interior wall were installed for a minimum concrete cover of 1/2 inch or less and accepted during QC inspection.
- 2. Bechtel Construction Specification CS-C02, Rev. 3, a documented instruction, states that the maximum standoff distance for cable tie down brackets in solid bottom cable trays shall be 1/4 inch.
Contrary to the above, on September 20, 1978, tie down brackets in solid bottom cable trays U21ABR3A, U21ABR6A, U21AASA9, U21AATA9, U21AASB1, and U21AATB1 were installed and accepted during QC inspection with a 3/4 inch standoff distance.

As a result, one bolt in.a tray splice plate was removed and not re installed during installation of the grounding straps.
