NOTICE OF VIOLATION Southern California Edison Company Docket No.

50-361 San Onofre Unit 2 License No. NPF-10 During an NRC inspection conducted on June 24 through July 28, 1993, a violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

10 CFR Part 50, Appendix B, Criterion XVI, requires that for significant conditions adverse to quality, measures shall assure that the cause of the condition isdetermined and that corrective actions are taken to preclude repetition.

Contrary to the above, corrective actions for inoperable main steam safety valves (MSSVs), a significant condition adverse to quality, in Unit 3 on July 15, 1990, were not adequate to prevent MSSV 2PSV8411 from being inoperable for a four day period ending February 8, 1993.

Procedural changes made in 1990 were not effective in preventing the uncontrolled removal of lagging from MSSV 2PSV8411 in February 1993 and resulted in the MSSV being inoperable.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Company is hereby required to submit a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Senior Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this-Notice of Violation (Notice).

This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violationI or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, the Commission may issue an order or a Demand for Information as to why the license should not' be modified, suspended, or revoked or why such other action as may be proper should not be taken.

Where good cause is shown consideration will be given to extending the response time.
