ENCLOSURE 1 NOTICE OF VIOLATION Southern California Edison Co. Docket Nos.: 50-361 San Onofre Nuclear Generating Station 50-362 License Nes.: NPF-10 NPF-15 During an NRC inspection conducted on January 12 through February 22,1997, two i violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:

i A. Unit 2 Technical Specification 5.5.1.1.a states that " Written procedures shall be established, implemented, and maintained covering the . . . applicable procedures 2

recommended in Regulatory Guide 1.33, Revision 2, Appendix A, February 1978."

Regulatory Guide 1.33, Revision 2, Appendix A, Section 3, recommends, in part, procedures for startup and operation of the shutdown cooling and emergency core 4 cooling systems.

Procedure SO23-3-2.6, Revision 10, Attachment 15, " Aligning Containment Spray Pumps as Standby SDC Pumps," Step 2.1.8, directs " LOCK (A) CLOSED S2(3)1204MUO62,CS Pump MP-012 Suction From Refueling Water Storage Tank isolation Valve," and Step 2.3.1, directs " LOCK (AB) OPEN S2(3)1201MU994,SDC to Containment Spray Suction isolation Valve (AB)."

Procedure SO123-0-20, Revision 4, "Use of Procedures," Step 6.2.12, directs that

" Procedure steps shall be followed in the exact sequence as written . . . ."

Procedure SO23-3-2.7.1, Revision 4, Attachment 10, Procedure Modification Permit 1, " Safety injection Tank Operation," Step 1.2, directs operators to maintain reactor coolant system pressure less than 350 psia.

Contrary to the above:

a. On January 29,1997, Unit 2 operators, while aligning Containment Spray Pump 2MPO12 as a standby shutdown cooling pump, did not follow the steps of Procedure SO23-3-2.6, Revision 10, Attachment 15, in the sequence as written. Unit 2 operators opened Valve S21204MU994 prior to fully closing Valve S21204MUO62, causing an inadvertent transfer of approximately 1000 gallons of water from the refueling water storage tank to the reactor coolant system, while the Unit was in Mode 5.
b. On February 13,1997, Unit 2 operators, while filling safety injection tanks, failed to maintain reactor coolant system pressure less than 350 psia.

This is a Severity Level IV violation (Supplement 1) applicable to Unit 2 (Violation 361/97002-03).

i B. 10 CFR Part 50, Appendix B, Criterion V, requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type l appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Procedure SO23-XXVil-4.34, Revision 0, "In-situ Hydro Test," Steps 4.9.3, 4.9.4, and 4.9.5, require, in part, to pressurize the steam generator tube and maintain the pressure for a minimum of five minutes.

Contrary to the above, on January 25,1997, during the in-situ pressure testing of the steam generator tubes, licensee craftsmen failed to maintain the pressure for a minimum of five minutes and maintained the pressure for one minute.

This is a Severity Level IV violation (Supplement I) applicable to Unit 2 (Violation 361;362/97002-02).

i Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Co. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this

, Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous doclwted correspondence, if the correspondence adequately addresses the i required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideratica will be given to extending the response time.

Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction. However, if you find it necessary to include such information, you should clearly indicate the specific information that you desire not to be placed in the PDR, and provide the legal basis to support your request for withholding the information from the public.
