Positive personnel access control was not maintained in accordance with Paragraph i 5.1.2 of the physical security plan, Revision 59, and Paragraph 6.6.2.1 of Security '

Procedure SO123-IV-4.4, Revision 5, " Security Lock and Key Control." Specifically, an i employee was allowed to enter the protected area with the wrong badge. An unreso'ved item was identified pending further NRC review (Section S1.1).
