00 APPENDIX A NOTICE OF VIOLATION Southern California Edison Company Docket No. 50-206 San Onofre Unit 1 License No. DPR-13 During an NRC inspection conducted on March 4 through April 15, 1991, as discussed during an enforcement conference on May 3, 1991, a violation of NRC requirements was identified. In accordance with 10 CFR Part 2, Appendix C, "General Statement of Policy and Procedure for NRC Enforcement Actions,"
1991, the violation is listed below:
Section 3.3.1.A of the San Onofre Unit 1 Technical Specifications, which applies to the operating status of the Containment Spray System, states, in part:
The reactor shall not be made or maintained critical unless the following conditions are met. In addition, the reactor coolant system shall not be increased above 200'F unless the containment spray system, the refueling water storage tank and the associated valves and interlocks are operable.
(2) Containment Spray System Two refueling water pumps are OPERABLE.
(3) Valves and interlocks associated with each of the above systems are OPERABLE.
Contrary to the above, containment spray flow limiter valve, S1-CRS-CV-518, was improperly coupled to its actuator on February 13, 1989, such that it was open when its actuator (including position indication for the valve) was in the closed position, rendering the valve inoperable. This valve remained inoperable until it was discovered on December 23, 1990; Unit 1 therefore operated with this valve inoperable throughout the cycle 10 operating cycle.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Senior Resident Inspector, San Onofre, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include: (1) the reason for the violation if admitted, (2) additional corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full 910618019n 9105306 0PDR,DC D
-2 compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the response time for good cause shown.
