ENCLOSURE 1 l

l NOTICE OF VIOLATION l Southern California Edison Co. Docket Nos.: 50-361 San Onofre Nuclear Generating Station 50-362 License Nos.: NPF-10 NPF-15 l

During an NRC inspection conducted on February 1 through March 14,1998, two violations of NRC requirements were identified. In accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:

A. 10 CFR Part 50, Appendix B, Criterion V, requires that " activities affecting quality shall be prescribed by documented instructions, procedures, or drawings . . . and shall be accomplished in accord &nce with these instructions, procedures, or drawings."

Procedure SO23-411-5712 " Engineering Procedure for the Installation of the Mechanical Nozzle Seal Assemb!ies for SONGS Units 2 and 3," Revision 3, Step 8.2.22, states that "After torquing of all threaded fasteners is complete, bend at least one tab of all Retainer Washers against the flats in its corresponding threaded fastener (either Hex Bolt or Hex Nut) and at least one tab against the adjacent component (either the Upper Flange or Top Plate). These Retainer Washers prevent loosening of their corresponding threaded fasteners."

Contrary to the above, on February 16,1998,16 retaining washer tabs on six completed mechanical nozzle seal assemblies were not bent against the flats in their corresponding threaded fasteners.

This is a Severity Level IV violation (Supplement 1) applicable to Unit 2 (Violation 50-361/98003-03).

B. The Physical Security Plan, Section 5.2.1.1, requires that all personnel who are not authorized unescorted access to the protected area (PA) shall be accompanied while in the PA by an individual who has been granted unescorted PA access, and that the escort shall be able to maintain ibual observation in order to detect any unauthorized i activities.

Contrary to the above, on February 16,1998, for approximately 1 minute, a contract person not authorized unescorted access to the PA was not being accompanied, while in the PA, by an individual who has been granted unescorted PA access. The assigned escort was not in a position to maintain visual observation in order to detect any l unauthorized activities.

l This is a Severity Level IV violation (Supplement Ill) applicable to Units 2 and 3 (Violation 50-361; 362/98003-06).

Regional Administrator, Region IV, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
