Collins, Regional Administrator, Region IV SUBJECT: COMPLIANCE WITH THE REQUIREMENTS OF 10 CFR PART 50, APPENDIX R, WATERFORD-3 - INSPECTION REPORT 50-382/84-20 On July 9,1984, Fegion IV issued to Louisiana Power & Light (LP&L) the results - ~ of a special safety inspection conducted at Waterford-3 (copy attached). Areas examined during the inspection included the implementation of the fire protection program and compli:nce with the requirements of 10 CFR Part 50, Appendix R, per FSAR commitments and SER evaluation. The inspection report indicates that the following open items have been referred to NRR for resolution.

SUBJECT: USNRC Inspection Report 50-382/83-03 Attached is a copy of USNRC Inspection Report 50-382/83-03 dated May 10, 1983. The report summarizes areas examined during an inspection conducted at Waterford 3 by Mr. L.E. Martin of Region IV on January 10-14 and January 24-28, 1983. During the inspection, three (3) Unresolved Items were identified and are contained in paragraphs 4a and 4b of the attached report. Unresolved items are matters about which more information is required in order for NRC to ascertain whether they are acceptable items, violations, or deviations.
Although no formal response is required to the USNRC for unresolved items, to provide a basis for timely closure of these items it is requested that you prepare a written response for each unresolved item stating what actions have been taken to resolve these matters and the date when this action was complete. Your response should clearly reference or enclose the records, documentation, reports, or information necessary to demonstrate resolution of the unresolved items.
If corrective actions are not completed, your response should state a date by which the corrective action will be complete.
It is requested that you forward your response to me by not later than April 15, 1983.

e Q'UALITY DEFICIENCY REPORT No. One Shast 2 of 3 , i OBSERVATION REQUIREMENTS: SAP-20, Orientation, Training, Qualification, and Checkout of Startup Personnel, Revision 3, 4/27/81.
Section 1.1 states in part: " Defines the requirements for the training and qualification of Startup and Plant Staff personnel involved in aramination, testing, and inspection activities. ." . Section 4.1 states in part: "All other Plant Staff shall utilize orientation from the Plant Operations Manual and from Department Heads."
Section 4.2.2 states in part: " Training in the following areas shall be required for personnel who perform inspection, aramination and testing activities that assure the quality of components in the Nuclear Power Plant..." (Esphasis Added)
Section 4.3.2.1 subsection (2) states in part " . shall have .. familiarity with the tools to be employed and demonstrated proficiency in their use."
Section 4.3.3.1 states in part: "The LSE shall ensure personnel obtaining (emptnsis added) and handling data or test results....., are certified to Level I standards as specified in this procedure."
FINDING: During a routine inspection, the NRC Resident Inspector T. Filipo,
j observed two LP&L personnel, A. R. Sneed and G. R. Rogers, attempting
to obtain vibration readings on a safety-related pump.
It was apparent
to the inspector that neither of the individuals were familiar with the task assigned as they did not know how to read the instrument. The Startup Engineer of the system M. George, was summoned and briefed j the individuals on the use of the instruments.
, l The NRC Inspector went to P. Prasankumar and informed him of observations
and concerns felt by inspector. Subsequent to visit of Frasaakumar, the NRC Inspector requested assistance of LP&L Startup QA. (Ref. QA Surveillance i Report SSW3 QA 83-85).
