3.2 The applicable technical manual for the. test equipment being used will be available for use during the performance of this procedure.
3.3 The equipment and cables to be tested using this procedure have been tagged in accordance with UNT-5-003.

.Maintonance Procodure ME-7-001 Insulation Resistance Revision 2 and Continuity Testing 3.4 Electronic and electrical devices that =ay be damaged by the test voltages to be used have been identified (by reviewing the applica-ble circuit diagrams) and disconnected from tne test circuits in ac-i i
cordance with SAP-10 or UNT-5-004, as applicable.
i 3.5 Circuits with parallel current paths are to be disconnected to the l
extent that no alternate current path exists other than the path to l
be tested.
I 3.6 All measuring and test equipment (M&TE) used to perform this procedure is within its current calibration cycle, as evidenced my an affixed calibration sticker.
37 Health Physics has issued a Radiation Work Permit (RWP) for work to be performed in radiation areas.
3.8 The area where testing is to be performed has to have a non-explosive atmosphere.
4.0 DDECAUTIONS AMD LTuTTATTONS 4.1 PRECAUTIONS 4.1.,1 Do not test any cables or items of equipment that are in a potentially explosive atmosphere.
If an area is suspect, have the atmosphere tested for explosive gases by the Safety Department.
4.1.2 Immediately before and af ter testing any equipment or cables (particularly in medium voltage equipment), connect a grounding strap first to ground, then to the test lead clips.
4.1.3 The word " ground" as used throughout the procedure refers to sta ti on resund, unless otherwise specified.
4.1.4 Ground straps should remain connected for at least four times the length of time required to perform the test.

Maintenance Procacure HE-7-001 Insulation P.esistance Revision 2 and Continuity Testing 4.1.5 During testing or maintenance, the equipment may remain energized; therefore, care shall be exercised to prevent electrical shock or equipment damage.
4.1.6 Post warning signs and barricade the areas in wnich exposed circuits will be energized by the test.
4.1.7 Never magger through motor bearings; i.e.,
from rotor windings to Case.
4.2 LIMITATIONS I
4.2.1 This procedure does not cover all cases that may be encountered
{'
during the performance of insulation resistance and and continuity testing; but, it does provide the basic safety precautions to be taken, the basic procedural steps for the tests, and the minimum acceptance criteria for each type of insulation resistance and continuity test.
The maintenance procedure for specific equipment having different acceptance criteria or procedural requirements shall take precedence over this procedure.
4.2.2 The input test' voltage snould not exceed the voltages described below unless stipulated by the Electrical Assistant Superintendent (E AS ).
4.2.2.1 Low voltage:
In order to avoid damage to 'certain low voltage equipment, the equipment is separated into two categories:
A.
Equipment that has an operating voltage of 0 to 250 Vac or Vec.
The test voltage should not exceed 250 Vdc.

Equipment that has an operating voltage between 251 and 600 Vac or Vdc.
The test voltage should not exceed' 500 Vdc.
4.2.2.2 Medium voltage:
Equipment that has an operating voltage between 601 and 15,000 Vac or Vdc.
The test voltage should not exceed
- 2,500 Vdc.

4.2.6 If any discrepancies are encountered which cannot be resolved within the scope of this procedure, submit a Condition Identification Work Authorization (CIWA) in accordance with SAP-08 l
or UNT-5-002, as applicable, to initiate further corrective action.
The CIWA number and an explanation shall be recorded in the " Remarks" section of the attachment.
4.2.7 The sections of this procedure are written in such a manner that they may be performed in any sequence or individually, if desired, f'
without affecting the end results of the procedure.

8.0 PPO CEDU RE 8.1 LOW VOLTA 0E CONTROL MOTOR TEST (Attachment 10.3)
I 8.1.1 Ensure that all prerequisites have been completed.
8.1.2 Measure the resistance from the motor housing to station ground (should be less than 1 ohm).
i NOTE i
If the motor to be tested is a single-phase motor, enter the data in the A4 block, enter N/A in the Bg and C/ blocks, and indicate it is a single-phase mocor in the " Remarks" sec-tion.
8.1 3 Measure the insulation resistance of each phase of the motor windings to station ground (should be > 500,000 ohms).
8.1.4 Disconnect the test leads from ground and the motor terminal.
8.1.5 Restore the motor to operational condition.
8.1.6 If the test results were satisf actory, have Operations clear any safety tags previously installed for the motor.
8.2 LOW VOLTAGE POWER AND MEDIUM VOLTAGE MOTOR TEST (Attachment 10 3) 8.2.1 Ensure that all prerequisites have been completed.

Maintenance Procedure ME-7-001 Insulation Resistance Revisien 2 and Continuity Testing CAUTION Prior to testing the resistance to ground, connect a ground strap between ground and the motor terminal (connecting the ground side first) to remove any static or capacitive charges.
After connecting the strap, allow a few moments for any charges to dissipate, then remove the strap.
8.2.2 Using an ohmeter set on a low resistance range, verify that there l
are no obvious shorts from any phase to ground and that continuity is exhibited between the phases.
8.2.3 Measure the resistance from the motor housing to the station ground (should be less than 1 chm).
8.2.4 Measure the insulation resistance of each phase of the motor winding to ground.
For motors smaller than 50 HP, the insulation resistance should be at least 2 megohms.
8.2.4.1 For motors 50 HP or larger, record the winding temperature'and correct all resistance measurements to 40 degrees C (see At-ta chment 10.5 for correction factor).
A.
On motors equipped with temperature sensors, the winding temperature may be obtained from the computer, or the motor and motor heater may be turned off and the equipment allowed to cool for sufficient time to reach. ambient temperature.
B.
Af ter the correction factor has beem determined, multiply it by the resistance values obtained in step 8.2.4 and enter the results in the blocks for the correctait insulation resistance.
The minimum accepta-ble insulation resistance shall be determined by using the equation:

Mascha Where Rm is the minimum insulation resistance in megoh=s at 40 degrees Celsius (104 degrees F) and kV is the machine rating in kV Rms.
8.2.5 Discharge the test voltage to ground until all residual voltage is dissipated.
8.2.6 Perform a polari:ation index test as follows:
8.2.6.1
'Using a megger (a motor-driven one is desirable), measure the resistance of one phase to ground for 1 minute and record the resistance reading.
Continue to measure the resistance for 10 minutes and record the resistance reading.-
8.2.6.2 Compute the polarization index (PI) by dividing the 10-minute reading by the 1-minute reading.
Record the result.
The minimum polarization index for the classes of insulation used are:

2.0 F
2.0 8.2 7 Disconnect the test leads from station gro'und and the motor terminal.
Discharge the test voltage to ground until all residual voltage is dissipated.
8.2.8 Restore the motor to operational condition.
8.2.9 If the test results are satisf actory, have Operations clear
- any safety tags previously installed.

8.3 LOW VCLTAGE CONTROL CABLE TEST (Attachment 10.2) l 8.3 1 Ensure that all prerequisites have been completed.
8.3.2 Lif t all conductors of the cable under test (including the shield wire or shielding).
NOTE Spare cond,uctors need not be tested (steps 8.3.3 through 8.3.5).
8.3.3 Using an ohmmeter, read and record the resistance to ground of-each conductor, including shielding (minimum:
500 kJh.)
8.3.4 Short all conductors together including the shielding.
- Then, isolate one conductor or shield and, using an chameter, measure the resistance between the still shorted conductors and the isolated conductor or shield.
Continue until all conductors have been measured (=inimum:
1 megohm).
8 3.4.1 Identify and record in the " Remarks" section of Attachment 10.2 tho'se conductors found to be shorted.
8.3.5 Separate conductors and shield from one another.
Use an ohmmeter to verify that all conductors (including shields) exhibit continuity between their end points.
8.3.6 Reconnect all lif ted conductors and shields to their termination pointa.
8.3 7 Using an ohmmeter, verify that the resistance to ground of the shield connection is less than 1 ohm.
8.3.8 Ensure that all grounding devices used for the test have been removed.

8.3 9 Have Operaticas clear any safety tags previously installed for the cable being tested, if the test results were satisfactory.-
8.4 LOW VOLTAGE POWER AND MEDIUM VOLTAGE CASLE TEST (Attachment 10.1) i 8.4.1 Ensure that all prerequisites have been completed.
CAUTION Sefore and after applying the test voltage, connect a grounding device between ground and the cable (s), connecting the ground side first, to remove any static or capacitive charges.-
After connecting the device, allow a few momenth for any charges to dissipate, then remove the device.
8.4.2 To test the cable, do one of the following:
8.4.2.1 Lift all conductors of the cable under test on both ends.
8.4.2.2 Isolate both ends of the cable from its and point by means of a breaker, switch or disconnect device.
8.4.2.3 Leave the cable attach.d to its and point and measure the cable and load together.
Results may have to be evaluated by the EAS to determine if the acceptance criteria have been met.
NOTE Spara conductors need not be tested (steps 8.4.3)'through 8.4.5).
8.4.3 Using a megehameter, read and record the resistance to ground of each conductor in the cable.

S.6.2 Measure the resistance from tne transformer to station ground (less than 1 chm).
CAUTION Before and after applying the test voltage, connect a grounding device between the ground and the winding terminal (connecting the ground side first) to remove any static or capacitive enarges.
After connecting the device, allow a few moments for any charges to dissipate, then remove the device.
If testing a polyphase transformer, each winding within the transformer should be grounded in this manner prior to any testing.
CAUTION I
Ground all windings of the transformer which are not being tested.

A.
On transformers equipped with. temperature sensors, the winding temperature may. be obtained from the meter on the transf ormer, or the transformer may be turned off and allowed to cool for suffic'ient time to reach ambient temperature.
B.
Multiply the resistance times the " Multiply By" figure (located.
straight across from the test temperature) as indicated on Attachment 10.5.
CAUTION Ground all windings of the transformer which i
are not being tested.
8.6.4 Measure the insulation resistance between windings within the transformer.
8.6.5 If required, perform a polarization index test as fo11cws:
8.6.5.1 Ground all. windings not under test.
l 8.6.5.2 Measure the insulation resistance to station ground of the tested winding for 1 minute, then 10 minutes, and record the readings.
8.6.5.3 Divide the 10-minute reading by the 1-minute reading and record this value.

8.6.6 Disconnect the test leads from station ground and the transformer terminals.
8.6.7 Have Operations clear any safety tags previously installed for the transformer being tested, if the results were satisf actory.

8.7 LOW VCLTAGE BREAKER / SWITCH TEST (Attachment 10.3) 8.7.1 Ensure that all prerequisites have been completed.
8.7.2 Using a megehmeter, measure the insulation resistance across each pole (line to load side) of the opened breaker.
8.7.3 With the breaker closed, measure the insulatien resistance between adjacent poles.
8.7.4 With the breaker closed, measure the insulation resistance f rom each pole to the breaker's metallic supporting structure (ground).
8.7.4.1 This test may be performed with the circuit breaker in place af ter the line and load conductors are removed, or with the circuit breaker removed and simulating in-service mounting at the test bench.

The inspection was conducted by Messrs. R. Hall and R. Rahl on June 19-21, 1984.
Inspection Report number will be 84-29.
2.
Mr. Hall closed the following SCD's.
SCD-68, " Spurious Actuation of ESFAS - J3109 Connectors" SCD-96 " Failure of CVCS Pump Train "A" to Start on SIAS" SCD-99, " Safety Injection Tank (SIT) Isolation Valve" SCD-97, " Diesel Generator "B" Turbocharger Failure (pending receipt of change 2 to MM-3-015)"
3.
Mr. Rahl closed the following SCD's.
SCD-102, " Dowels and Shear Blocks" SCD-103, "Radflex Wall Penetrations" SCD-88, "RWSP Nozzle / Liner Overstress (pending receipt of calculations from Ebasco)"
4.
Mr. Hall recommended amending the final report of SCD-96 due to the' further evaluation that has lessened the safety significance of the electro switch relays. He also, commented on.the lack of material failure analysis on the gears of the Safety Inspection Tanks Isolation Valves. He further, stated he was looking at NCR 7680, pertaining to SCD-74, which was closed on inspection 84-24.
He will inform LP&L if any problems are generated by this NCR review.

PLANT PROTECTION SYSTEM MODIFICATION

During May of 1983, Electro-Mechanics, Inc., performed a modification to the plant protection system at Waterford under CIWA 834978 incorporating DCN-IC-1375. The modification procedure required that the technicians initial Tables I and ll as the wiring changes were made.
However, there was not an obvious place to initial and, as a result, initials were omitted.
Thit error has been corrected by Electro-Mechanics and a statement explaining the omission was added by the Electro-Mechanics QA/QC engineer. CE has reviewed the attachment and finds the corrections acceptable. It is recommended that the attachment be added to the work package.
