Introduction:==
This report is submitted pursuant to 10CFR 50.55(e).
It describes a deficiency in the Measuring and Test Equipment (M&TE) program. This problem is considered repcreable under the requirements of 10CFR 50.55(3).
This problem has not been identified to the USNRC pursuant to 10CFR21.
==
Description:==
Numerous M&TE usage sheets, used in support of the tect program, were either not forwarded to the proper organization or not posted against test equipment use records for traceability of individual M&TE usage.
Safety Implications:
If left uncorrected, failure to have traceability of M&TE usage could result in an instrument being found out of calibration and not having the ability to evaluate the effect on completed plant tests.
Following is Startup's final response input to SCD No. 98.
Corrective Action:
(1) M&TE usage sheets transmitted since Decceber 1983 have been transcri'aed into the M&TE Record of Use Log.

(2) A search of M&TE use records and test procedures (prior to December 1983) has been completed which evaluated the extent of missing M&TE use records and the accuracy of existing M&TE use records.
(3) An evaluation process for determining the impact of the out-of-tolerance condition on safety related testing has been completed.
Evaluation Findings:
A total of eighty-nine (89) tests were evaluated which included all safety related tests and a few tests which were deemed to be important to safety.
The number of instances where M&TE was checked out to be used for the above tests was 1264. Of these instances only 5 times existed where traceability of M&TE equipment, or evaluation of out-of-tolerance resulted in actual retest. Additional testing will be performed, for those specified cases, to ensure adequacy of data. Retesting is being followed by the LP&L, Condition Identification Work Authorization (CIWA), and the CIWA results will be reviewed and approved by the Joint Test Group.

1.
Not getting to the naistenance sheps at all, 2.
Not being routed to the applicable shop (MM, ME, MI) when they do reach naistanance, 3.
Not all equipnent used in a test inre appearing on the M&TE sheet; 4.
Sheets are not getting to the shops in a cinely nanner.
O To resolve these concerns the following vill be followed:
1.
STP's, C17A Ratests, SFG's and Preops require the use of M&TZ sheets. All Startup Engineers will ensur,e M&TE sheets are made out when test equipment is used under the cover documents above. The M&TI sheets vill be nade out in a timely manner (by the end of the shif t in which the test equipnent was used) and given to Sonny Bradley.
2.
Sonny Bradley v111 route the M&TE sheets to A. Valenzuela each. Friday.
Please apply your attention to the above concern and avoid any futura problems with the M&TE sheets.
