The violation cites the 47' minute delay; however, the GPU Nuclear response that follows also addresses the delay in activation of the Emergency Response Facilities (ERF). -

1. Reason for Violation A. Delay in Initiating the Callout .

The delay in initiating the callout was caused by the on shift Emergency Director's consideration that there was no plant instability requiring the immediate support of the off shift ERO and the lack of contingency plans in place to provide safe access to the >

on site ERF when normal access was restricted.

1 Page 2 of 2 B. Delay in Activation of the ERF 1. One element of the delay in activation of the ERF was locking the Control Room fire doors, as required by procedure,-

preventing access to the shift supervisor's' office. Maintaining ,

the Control Room fire doors locked prevented access to the Emergency Preparedness (EP) pager callout phone and the EP pager call back lines, located in the shift supervisor's Office.

2. The remaining element of the activation delay was the need to manually call each member of the ERO individually rather than using the automated callout system which procedurally could only be accessed from the shift supervisor's office. Backup plans to activate the ERO pagers and place a message on the call back line had not been proceduralized. ,

II. Corrective Steps That Have Been Taken and the Results Achieved A. Delay in the Callout Procedure EPIP-TMI .02 has been revised to proceduralize the >

requirement that callout of the ERO is to be initiated in' parallel with or immediately following notification of offsite agencies. The Emergency Report Form has been revised to include action to address emergency situations that restrict access to the onsite Emergency Response Facilities. Guidance has also been provided to the Emergency Directors, Emergency Support Directors and Shift Supervisors on how to select alternate routes of access when the ,

primary routes are not available.

i B. Delay in Activation of ERF 1. The locking scheme of the Control Room fire doors contained in I Procedure 1202-13 has been revised to allow continuous access from the control room to the shift supervisor's office, the EP pager callout telephone, and the EP pager call back line. ,

i 2. Procedure EPIP-TMI .04 has been revised to provide instructions for group pager activation and placing a message on the call back line if the shift supervisor's office becomes inaccessible.

This eliminates the need to rely on individual callouts.

III. Corrective Steps That Will Be Taken To Avoid Further Violations Actions to correct the causes for this violation have been presented above. No further corrective actions are required. However, further drills and training will reinforce the above revisions and concepts.

IV. Date of Full Compliance GPU Nuclear believes that all corrective actions have been implemented.
