Carolina Power & Light Company (CP&L) hereby submits the 1986 Annual Report on Primary Safety and Relief Valve Challenges as required by Technical Specification 6.9.1.b.3.

On two separate occasions during 1986, the primary power operated relief valves (PORVs) were challenged. There was no challenge to a primary safety valve during 1986. Details on the challenges are provided by the enclosure.

1. EVENT: CHALLENCE TO PRIMARY RELIEF VALVES DATE: JANUARY 28, 1986 EVENT DESCRIPTION: The block valves to both pressurizer PORVs, PCV-455C and PCV-456, were closed due to excessive PORV leakage. While in this condition, the reactor tripped on high Reactor Coolant System pressure, following a loss of offsite power (LER 86-05). PCV-455C was defeated because of loss of power to its controller. Although the valves were isolated and one, inoperable, they received or should have received a signal to open during the pressure increase. This is considered a challenge by definition (NUREC-0737).
2. EVENT: CHALLENCE TO A PRIMARY RELIEF VALVE DATE: SEPTEMBER 2, 1986 EVENT DESCRIPTION: With the RCS in cold shutdown but pressurized, a second charging pump was inadvertently started. PCV-456 opened twice before operator action could reduce system pressure below the LTOPP setpoint (30-Day Special Report, RNPD/86-4554).
