SUBJECT: REPORT NOS. 50-325, 324/88-15 AND 50-325, 324/88-18

Thank you for your response of August 24, 1988, to our Notice of Violation and

Proposed Imposition of Civil Penalty issued on July 25, 1988, concerning  ;

activities conducted at your Brunswick facility.

We have evaluated your response and have found it to be incomplete. Our letter

' specifically stated that "The significant issue manifested by the collective

violation is operator error and inattention to detail, an issue of critical

importance to the safe operation of your plant," and that "The NRC staff

-

considers the lack of attention to detail by the licensed operators during

these events to have significant safety implications which must be corrected

irmiediately. Management must evaluate this apparent trend, identify the root

cause, and implement corrective actions." We have determined that, although

you have addressed the individual violations, your response did not discuss the

resolution of the rcot cause problem manifested by the three violations.

Based on a conversation on September 20, 1988, between P. E. Fredrickson of my

staff and J. L. Harness of your staff, we understand that you will provide a

supplement to your August 24 response by September 30, 1988, addressing the

root cause resolution for the identified operator inattentiveness trend. Our

evaluation of your response to the Proposed Imposition of Civil Penalty is

, still under review and will be addressed through separate correspondence.

We appreciate your cooperation in this matter.
