EMERGENCY DIESEL GENERATORS AND THE HIGH PRESSURE

COOLANT INJECTION SYSTEMS (NRC INSPECTION REPORT

50-233/92-31) {

This letter re'ers to the Probabilistic Risk Assessment Based Inspection conducted by

Mr. James Trapp and other NRC staff on November 30 through December 18,1992. The

inspection included a review of activities authorized for your Pilgrim Nuclear Power Station, l

in Plymouth, Massachusetts. At the conclusion of the inspection, the team findings were

discussed with you and other members of your staff on December 18, 1992. The purpose of

this inspection was to verify that the emergency diesel generators and the high pressure 4

coolant injection system were being operated, maintained, tested and modified in a manner

which ensured that the systems were reliable and capable of performing their intended safety

function if called upon to operate.

Areas examined during the inspection are discussed in the enclosed inspection report. Within

these areas, the inspection consisted of selective examinations of procedures, installed plant

modifications, equipment maintenance, conduct of surveillance testing, operator performance,

and observations by the inspectors. The emergency diesel generators and the high pressure

coolant injection system were selected for this inspection based on their relatively high

. importance to safety as described in Pilgrim's probabilistic risk assessment. Based on the

information reviewed, the team concluded that the emergency diesel generators and the high

pressure coolant injection system were being operated, maintained, tested, and modified in a

manner which assured operability and reliability.

Th. team noted that efforts were being made to improve reliability. These efforts include

modifications to the emergency diesel generator starting air and turbo-boost air systems.

'

Strengths were noted in the functional testing of the emergency diesel generator logic relays

and the inservice testing of valves. Licensed operator performance observed during

surveillance testing and walkdown of in plant procedures was strong. - Overall, the team j

concluded that controls provided for the operation, maintenance, surveillance and design

i

changes for the systems reviewed were technically sound and thoroughly implemented.

Based on the results of this inspection, two of your activities appeared to be in violation of

NRC requirements, as specified in the enclosed Notice of Violation (Notice). The first

violation was the failure to adequately control the position of a ventilation damper located in  :

'

the emergency diesel generator building. A manually operated exhaust damper was identified

by the team as being closed when the damper should have been open. The second violation

was the inadequate corrective action taken in response to repetitive emergency diesel

generator fuel oil booster pump V-belt failures. The V-belts have been periodically failing

since the emergency diesel generators were installed, with the last failure occurring dudng

this inspection. While the interim corrective action has been somewhat successful in

reducing the belt failures, the permanent corrective action to improve the fuel oil booster

pump design had been delayed. Your response to this Notice should follow the instructions

provided in the Appendix.
