Dear Mr. Fiorelli:

INSPECTION REPORT NO. 50-483/80-22 This is in response to your letter of April 2,'1981, indicating that IE:HQ had confirmed the validity of non-compliance item 483/80-22-01, regarding the safety-related classification of the Ultimate Heat Sink rip-rap. On April 24, 1981 Mr. Konklin granted a one week extension in the response due date.

The following is in response to item 1 in Appendix A, Notice of Violation, of inspection report 50-483/80-22 transmitted with your letter dated September 29, 1980.

Infraction (483/80-22-01) 10CFR50, Appendix B, Criterion II states, in part, that "The applicant shall identify the structures, systems and components to be covered by the Quality Assurance Program."

The FSAR Sectivn 17.1.2 states, in part, "The Quality Assurance Program is to be applied,to those structures, systems, equipment and components that are necessary to assure the integrity of the reactor coolant pressure boundary, the capability to shutdown the reactor and maintain it in a safe shutdown condition, or the capability to prevent or mitigate the consequences of accidents which could result in potential offsite exposures..."

Contrary to the above, the rip-rap on the ultimate heat sink is not listed in Table 3.2-1 of the FSAR as safety related. Further-more, material cest results could not be located while on site. The rip-rap appears to

.ie placed improperly, in that it is not graded l and there are pockets of fine material on the I

surface.

Response

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED Rip-rap on the Ultimate Heat Sink will be listed in Table 3.2-1 of the FSAR as safety related.

Material test results have been located and are available for inspection at the site.

Our geotechnical consultant, who inspected the completed rip-rap installation, identified in a preservice inspection report some areas of rip-rap that were considered either unacceptable or marginally acceptable. These areas were reinspected by the constructor and reworked as necessary. The con-sultant reinspected the rip-rap installation and the reworked areas and with Union Electric's concurrence accepted the rip-rap and deemed it adequate.

CORRECTIVE ACTION TO BE TAKEN TO AVOID FURTHER NONCOMPLIANCE Rip-rap for the Ultimate Heat Sink will be listed in Table 3.2-1 of the FSAR as safety related. Specifica-tion No. 10884-C-001 for the rip-rap, will be revised to a "Q" specification.

THE DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance is expected to be achieved by June 15, 1981.

In paragraph 2 of Mr. Keppler's letter to me dated April 2, 1981, it is stated that we should include slope protection inspection of the UHS as a FSAR commitment. Table 2.4-14, sheet 3 of 5, paragraph 2.a.5 of the FSAR contains our commitment for slope protection inspection and we are proceeding on this basis.
