SUBJECT: NRC SPECIAL INSPECTION REPORT NUMBER 50-483/99-11
Dear Mr. Randolph:
This refers to the special inspection conducted on August 12-20,1999, at the Callaway Plant.
The inspection focused on the causes, circumstances, and corrective actions associated with
the August 11,1999, rupture of a 6-inch drain line between a moisture separator reheater first
stage reheater drain tank and feedwater Heater 6B.
Following the event, operator performance in shutting down the unit was good. The team found
that a weakness in your flow-accelerated corrosion monitoring program resulted in no previous
inspections of the pipe segment which failed. The enclosed report presents the results of this
,
inspection.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure, and your response, if requested, will be placed in the NRC Public Document Room.
