ENCLOSURE NRC INSPECTION REPORT NOS.

50-259/81-03 ) 50-260/81-03,'ND 50-296/81-03 BROMNS FERRY NUCLEAR PLANT REVISED RESPONSE TO R. C. LEMIS'ARCH 10, 1981 LETTER TO HE G. PARRIS Item B 10 CFR 50, Appendix B Criterion IX and Topical Report TVA-TR75-1, paragraph l7.2.9 requires in part that special processes, includirig welding, shall be accomplished by qualified personnel.

Contrary to the above, 10 CFR 50 Appendix B Criterion IX'and Topical Report TVA-TR75-1 which requires welding to be accomplished by qualified personnel was not met in that on January 15, 1981, safety-related welding was performed on workplan 7779R1, Installing of Hydrogen-Oxygen Monitoring Panel, in which the q'uaiifications of the welder could not be determined.

This is a Severity Level V Violation (Supplement II.E.1) applicable to unit 3.

Admission or Denial of the Alle,ed Violation The violation is incorrect as s'tated. The subject workplan did contain

'a .List of welders performing this work and certification of these welders had been confirmed. Therefore, all of the welds were performed bv certified welders.

However, we do agree that a violation did occur because the workplan did not contain sufficient information at the time of,inspection to determine wl ich individual welder performed each of the welds in question.

Reasons for the Violation .if Admitted On January 15, 1981, the subject workplan was being reviewed by the Quality Assurance Staff and the cognizant engineering personnel. At that time the resident NRC inspector noted that the welder identification number was not listed on some specific welds in the weld data sheets.

This occurred because of the failure of responsible 'individuals to properly consolidate welding data and welding information during field work.

Corrective Ste s Which Have Been Taken and the Results Achieved The affected weld data sheets were reviewed-and consolidated by Quality Assurance and cognizant engineering personnel. In cases where .the welder identification number was missing, interviews were held with the craft foremen and craftsmen who actually performed the work to determine this information. The weld data sheets were formally signed off as complete by the 'Quality Assurance Staff on January 27, 1981. This included verification that the welders listed for specific welds were certified.

Corrective Ste s Which Mill Be Taken to Avoid Further Violations The Weld Data Sheet for non-radiograph welds in division procedure No. N73M2 has been revised to include a QC inspector verification of welder certification. The inspector's signoff indicates that the welder for each weld is correctly entered on the data sheet and the welder is qualified.
