Reference 1 transmitted a summary report (dated March 1989) on the evaluation of BFN cable installation concerns. Reference 2 transmitted a summary report which addressed the recommended actions contained in the March 1989 report l
required to resolve BFN's cable issues. Additionally, Reference 2 contained a i
summary of the remaining actions required for closure of BfN cable installation concerna except for cable pullby. The cable pullby program is now complete and the report transmitted by Reference 2 has been revised to provide the results of this prog am.
Additionally, this report has been revised to clarify cable bend radius and support of vertical cable, and to document resolution of BFN's januning issues with NRC.
It should be noted that during the hi-pot testing being performed to resolve the cable pullby issue, cable damage was identified.
However, the. damage was
-attributed to a disconnected cable bushing at a mio point pull box (JB) and not cable pullby. Subsequent to the discovery of this anomaly BFN established a program to identify conduits at JBs with missing bushings.
contains the results of the walkdown and subsequent hi-pot testing that was performed to resolve this issue. Conduits'at JBs with missing bushings were identified during BFN's walkdowns, but the hi-pot testing and investigations of the ef fected cable indicated that cable damage that was originally identified during the cable pullby testing program was an isolated case.
