APPENDIX A Suncrux, Incorporated 10440 North Tantau Avenue Cupertino, California 95014 License No. 04-17679-01 E Notice of Violation Based upon the results of the NRC inspection conducted on October 25, 1978, activities under your license appear to be in noncompliance with NRC requirements as identified below.
A. License Number 04-17679-01E authorizes the distribution of American Atomics Model 60297 sealed sources in watch module numbers DM400 or DM500. Distribution of other sources or watch modules is not authorized by the license.
American Atomics Sealed Source Models 60307, 60320, and 10726, and watch module R4600 were used in watches distributed between November 15, 1977, and October 25, 1978.
This is an infraction.
B. License Number 04-17679-01E requires that all source, module, and watch case combinations that are distributed undergo specific prototype tests which are delineated in the license.
Prototype tests for watches described in Item A have not been conducted.
This is an infraction.
C. License Condition 1.C states that "Watchcases shall be permanently marked to indicate the presence of tritium as authorized by specific amendment to this license. Watches shall also be per-manently marked to identify the manufacturer."
Watches distributed between November 15, 1977, and October 25, 1978, were not marked to identify Suncrux as the manufacturer, and the presence of tritium was not indicated as shown in Drawing SK 10777 in the application dated October 5,1977.
This is a deficiency.

Y Appenaix A (Con't.) . D.Section XV(B) of Suncrux procedures submitted as part of the license application dated October 5,1977, and referenced in the license requires that records of quality assurance test-ing be maintained for review by the NRC.
No records of the results of the quality assurance tests were available for review at the time of the inspection.

E. 10 CFR 32.25(c) requires that an annual report describing transfers of byproduct material made pursuant to 10 CFR 32.22 be filed with the NRC.
