APPENDIX A NOTICE OF VIOLATION
.RSL 2015 East University Drive Carson, California 90746 During an NRC-inspection conducted on July 17, 1989 a violation of NRC requirements was identified.
In accordance with the " General Statement of Polic and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988, the violation is listed below:
A.
10CFR30.18(b)provides,inpart,thatnopersonmay,forthepurposes of commercial distribution, transfer byproduct material in the individual quantities set forth in K30.71 Schedule 8, to persons exempt under this section or equivalent regulations of an Agreement State, except in accordance with a license issued under K32.18 of this chapter, which license states that the byproduct material may be transferred by the licensee to persons exempt under this section or the equivalent regulations of an Agreement State.
Contrary to the above requirement, RSL had been distributing iodine 125 sources manufactured by Reflex Industries to pusons exempt from licensing pursuant to 10 CFR 30.18(b) or equivalent provisions of the regulations of any agreement state.
This is a Severity Level IV Violation.

As of June 20, 1989, all byproduct materials were returned to Reflex Industries. No further transfers occurred. Subsequently, an application for an exempt-distribution license was submitted to NRC Headquarters.
Pursuant to the corrective action taken by the licensee relative to the above-violation, no further response to this item is required.
