Southern California Edison Company Docket Nos. 50-206 San Onofre Nuclear Generating Station License Nos. DPR-13 Units 1 During an inspection conducted on March 13 through April 3,1989, a violation l-of NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988) as modified by 53 Federal Register 40019 (October 13, 1988), the violation is listed below:
10 CFR 19.12, Instructions to workers, reads in part:
"All individuals working in or frequenting any portion of a restricted area shall be kept informed of the storage, transfer, or use of radioactive materials or of radiation in such portions of the restricted area,..., in precautions or procedures to minimize exposure...The extent of these instructions shall be commensurate with potential radiological health protection problems in the restricted area."
Contrary to the above, on December 22, 1988, a worker was not informed of the storage of radioactive material and the radiation levels in a portion of the Steam Generator "A" platform and was not instructed in the precautions and procedures necessary to minimize his exposure.
As a result, the worker received an unplanned exposure to his left arm of approximately 1600 mrem.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Company is hereby required to submit a written statement of explanation to the U.S.
Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and h copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is'not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
Consideration may be given to extending the response time for good cause shown.
