IIIC,h ketNo.:nitName':CompletedBy:Telephone:Date:Page:50-316.D.C.CookUnit¹2.R.S,Lease('616)465-5901August7,1980Page2of37/19/80--TheUnittrippedat0004hoursduetoLow-Lowlevelin¹1SteamGenerator.ThiswascausedbyreducedflowfromtheEastMainFeedPumpwhichwasseentobelosingspeed.Powerlevelattimeoftripwasnear100'A.Anattemptwasbeingmadetoreduceloadingtomatchthereducedfeed-waterflow.EarlyanalysisofthefeedpumpproblemwasthatsteampressureregulatingvalveARV-ll,whichsuppliessteamtothefeedpumpturbine,hadmalfunctioned.Thoroughcheck-outofthefeedpumpturbineanditssteamsupplycouldnotidentifyanyproblem,andthispumpwasreturnedtoservicewiththeUnit.TheReactorwasreturnedtocriticalityat0439hours.4TheTurbineGeneratorUnitwasrolledat0626hoursandplacedinparallelwiththesystemat0757hours.TheUnitwasloadedto50!powerby1159hoursandheldatthispointwhilecheck-outoftheEastMainFeedPumpwastakingplace.Unitloadingwasinitiatedat1740hours.7/20/80--Unitwasloadedto100$powerby0355hours.ValveIMO-910(suctionfromRefuelingWaterStorageTanktoChargingPumps)wasinoperableforan11.75hourperiodwhenthevalvefailedtostrokewithpower.7/21/80--ContainmentRadiationMonitorsR-11andR-12wereinoperablefora7hourperiodwhilerepairsweremadetothefilterpaperdrive.7/23/80--TheEastResidualHeatRemoval,Systemwasinoperablefora17hourperiodforpreventivemaintenance.7/25/80--TheFireDetectionSystemofEngineeredSafetyFeatureVentilationCharcoalFilterHV-AES-1wasinoperablefora3.5hourperiodfornecessaryrepairs.

DOCKETNO.UNITNAMEDATECOMPLETEDBYTELEPHONEPAGE50-316D.C.Cook-UnitNo.2~8-6-80B.A.Svensson616465-5901MAJORSAFETY-RELATEDMAINTENANCEJULY,1980No.1steamgeneratorblowdownregulatingvalve,DRV-312,hadabodytobonnetleak.Lappedthevalveseatsandreplacedallthegaskets.Hadvalvetested.M-2CSI-lAweldleakwasdetecteddownstreamofanewlyinstalledtestvalveonthedischargeofthemotordrivenauxiliaryfeedpump.The1"socketweldconnectionwasreweldedandNDEwascompleted.Loop3protectionchannelIIHighSteamLineFlowstatuslightwasperiodicallyreceived.MFP-130calibrationwasperformedandreturnedtoservice.CSI-2ILA-121,accumulatorlevelindicationwascyclingbetween50and60percent.TheredundantchannelILA-120wasremainingconstant.TransmitterILA-121wascalibratedandreturnedtoservice.Normalcontrolroomindicationwasrestored.CRI-3C&I-4FRV-240,steamgeneratorNo.4feedwaterregulatingvalve,wasstickinginvariouspositions.Thevalveactuatorwasreplacedandpropervalveoperationverified.Theaxialpowerdistributionmonitoringsystemfailedtoprintthedata.Thepowersupplyfortheprinterhadfailed.Asparepowersupplywasinstalledandnormaloperationoftheprinterwasverified.
