ENCLOSURE 1 PLANT HATCH - UNIT 1 NRC DOCKET 50-321 OPERATING LICENSE DPR-57 VIOLATION 89-04-01 AND GPC RESPONSE I-t VIOLATION 89-04-01 Technical Specification 6.8.1.a requires that written procedures be established, implemented, and maintained as recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978.

Section 4 of Appendix "A" of Regulatory Guide 1.33 recommends procedures for operation of the Reactor Hater Cleanup System.  ;

Procedure 34S0-G31-003-1S, " Reactor Water Cleanup System," provides written instructions for operation of the Unit 1 Reactor Water Cleanup System.

Contrary to the above, procedure ~34S0-G31-003-1S was inadequate in that it did not caution against the consequences of system cooldown '

or provide adequate instructions to prevent rapid system filling. As a result,.on February 4, 1989, while operations personnel were  ;

warming the 1A Reactor Water Cleanup (RHCU) system pump prior to  ;

placing it in service, an unanticipated isolation of the Group 5 l Primary Containment Isolation System valves occurred due to a high i differential flow condition in the RHCU system. At the time of this I

. event Unit 1 was operating at approximately 100 percent of rated power.

This is a Severity Level V violation (Supplement I). .

VIOLATION 89-04-01 AND GPC RESPONSE Reason for the violation:

The violation was caused by an inadequate procedure. Procedure  !

34S0-G31-003-15, " Reactor Hater Cleanup. System," did not provide adequate instructions in the pump warming section.

Specifically, the procedure did not caution against the possibility of partial voiding as the system pressure and .i temperature decrease and did not provide adequate instructions to prevent rapid system filling.

Corrective steos which have been taken and the results achieved:

As a result of this event, the RHCU system was re-filled and re-pressurized. .The "A" RHCU pump was then warmed and placed successfully into operation. Procedures 34S0-G31-003-IS and 34S0-G31-003-2S were temporarily revised on 5/1/89 to correct the identified deficiencies.

Corrective steos which will be taken to avoid further violations:

The Unit 1 and Unit 2 RHCU system operating procedures will be 1 revised permanently by 6/30/89. l Date when full comoliance w?ll be achieved:

Full compliance was achieved on 5/1/89 when the Unit 1 and Unit 2 RHCU system operating procedures were temporarily revised.
