This inspection covered activities authorized by NRC Construction Permits CPPR-126 and CPPR-127 for Comanche Peak Steam Electric Station Units 1 and 2. We have reviewed your letter dated August 5,1987, concerning the inspection conducted by Mr. I. Barnes and ot'1er inspectors in the Comanche Peak Project Division during the period from March 1 through April 30, 1987. This inspection covered activities authorized by NRC Construction Permits CPPR-126 and CPPR-127 for Comanche Peak Steam Electric Station Units 1 and 2. Attached to your letter was a Notice of Violation and Deviation.
We hereby respond to the Notice of Violation and Deviation in the attachment to this letter.

_(_44 5/8706-V-12 : 446/8705-V-06) l Criterion V of Appendix B'to 10 CFR Part 50, as implemented by Section 5.0, l Revision 3, dated July 31, 1984, of the TV Electric Quality Assurance Plan, j requires that activities affecting quality shall be prescribed by and j accomplished in accordance with documented instructions, procedures, or l drawings. i Section E-6.5 of Brown & Root Procedure-MCP-10, Revision 9, " Storage and Storage Maintenance of Mechanical and Electrical Equipment," requires that "All colored cables stored in type class. "E" storage shall have the cable covered for protection against exposure to sunlight (i.e., replacement of shipping cardboard or similar material)."
Contrary to the above, on March 12, 1987, the NRC inspector identified colored 4 cables stored in the outside (type class "E") cable reel yard which were- !
improperly protected from exposure to sunlight. The opaque plastic covering -
installed to protect the cables from sunlight failed to cover a significant ;
portion of the cable. These cables were noted to have begun fading in the '
exposed area. Examples of the above condition were noted on cable reels W-416-1, W-316-1A, W-745-1CC, W-117-1, W-117-3, W-117-4, and W-118-12C.

The Cable Reel Yard Area Supervisor has been directed to perform a daily surveillance which includes observation of protective covers. This surveillance will be performed in addition to the established program of weekly independent surveillance by the Warehouse Trairiing Coordinator and random ;
surveillance by the Warehouse General Supervisor.

A. TV Electric letter TXX-4826 dated June 16, 1986, states, in part, with respect to action to prevent recurrence for violation 445/8516-V-13,"The support addressed was inspected in 7/83 in the earliest versions of the As-built Inspection Program. Due to problems related to undersized welds, B&R Procedure QI-QP-11.1-28, Revision 24 issued April 24, 1984, was revised to require the QC inspector to assign a weld number to each weld .
shown on the design drawing and to doctment both the required size of the !
weld per design drawing, and the actual deposited weld size. All QC personnel were trained in the procedure revision and inspection methodology . . . ."
In deviation from above, Revision 24 of B&R Procedure QI-QAP-11.1-28 was not revised to require the QC inspector to assign a weld number to each weld shown on the design drawing and to document both the required and actual deposited weld size. These requirements have existed virtually unchanged since at least Revision 16 dated December 15, 1982, which was before the applicant inspection of the identified support in July 1983 (445/8706-D-03).

TV Electric agrees with the alleged deviation and the requested information follows: ,
- 1. Reason For Deviation i
The response to violation 445/8516-V-13 was incorrect due to inadequate research, preparation and review of the response by QA Department personnel prior to its submittal.
- 2. Corrective Steos Taken And Results Achieved TV Electric responses provided by the QA Department for open NRC non- l compliances have been reviewed. One other item for which a revised response was required was identified and a revised response was submitted on TXX-6473 dated May 29, 1987. The other responses reviewed were verified to be adequate. A revised response to violdion 445/8516-V-13 will be submitted by October 9, 1987.

- 3. Corrective Steos Which Will Be Taken To Avoid Further Deviations '
In April 1987, TV Electric instituted measures within the QA Department to assure that NRC responses from the QA organization were clear and accurate. The measures consisted of staffing the Issue Interface i Coordinator position. The Issue Interface Coordinator is responsible for independently verifying the content of TV Electric responses which i originate in the QA Department. l In addition to the corrective actions described above, a Quality Assurance Department procedure (NQA 4.01, "NRC Issue Interface,") has been prepared. I This procedure will formalize the administrative controls applicable to the Issue Interface Coordinator and ensure adequate preparation and review of responses made by QA Department personnel.
- 4. Date When Full Comoliance Will Be Achieved l Procedure NQA 4.01 will be issued by September 18, 1987. Full compliance will be achieved by October 9, 1987 with the issuance of the revised j response to violation 445/8516-V-13.

TV Electric letter TXX-4740 dated April 3,1986, committed in response to deviation 445/8513-D-03 to have the recently instituted ERC overinspection group repeat a number of completed reinspection which.would include a review for blatant out-of-scope observations. A determination would be 1 made, based on the findings of the overinspections, as to whether j additional reinspection and/or inspector training on documenting out-of- j scope observations would be required. TU Electric committed to be in full l compliance by April 30, 1986.
In deviation from the above, no evidence was made available to confirm that overinspection out-of-scope findings had been utilized to make a ,
determination of whether additional reinspection and/or inspector training would be required (445/8706-D-02).
RESPONSE TO NOTICE OF DEVIATION ITEM B (445/8706-D-02)
TV Electric agrees with the alleged deviation and the requested information follows:
- 1. Reason For Deviation The response to deviation 445/8513-D-03 was incorrect due to a miscommunication internal to CPRT.
The CPRT failed to implement the review for blatant out-of-scope observations by the overinspection group because the QA/QC Review Team, who had the responsibility for implementing this review, never became aware that a commitment to conduct the review had been made. Furthermore, .
upon further reflection, it becomes obvious that it would have been very I difficult if not impossible to obtain meaningful quantitative results from a comparison of out-of-scope observations recorded by the overinspection group to those recorded by the inspectors who performed the reinspection.
This is a result of the nebulous nature of the responsibility that inspectors performing the reinspection had regarding the observations of l out-of-scope deviations.
- 2. Corrective Steps Taken And Results Achieved The described deviating condition will be rectified with a revised response to deviation 445/8513-D-03, as was previously committed to in TV Electric letter TXX-6342 dated May 5,1987.
Additionally, in April 1987 CPRT initiated a review of responses to the '
NRC from CPRT to assure that committed actions were adequate and were being implemented by the individuals responsible for the committed actions.
