The information is as initially received without verification or evaluation, and is basically all that is known by the Region IV staff on this date.

SUBJECT:

MAJOR STEAM LEAK RESULTING IN PLANT SHUTDOWN On July 8,1986, at 12:25 CDT, the licensee experienced a reactor scram due to a main steamline isolation valve (MSIV) closure while at 65 percent power.

This isolation initiated due to turbine building high temperature resulting from a manway gasket failure on a moisture separator reheater (MSR) drain tank. The plant responded as expected and no emergency core cooling system (ECCS) initiations were experienced. During the initial transient, two safety relief valves (SRV) lifted, limiting reactor peak pressure to 1086 psig. The reactor core isolation cooling (RCIC) system was placed on line to control reactor pressure. Reactor water level was controlled using the reactor feed pump (RFP). . Plant personnel were evacuated from the turbine building and no injuries occurred. The unit is presently shutdown while the licensee investigates the cause of the manway gasket failure. The resident inspectors will be monitoring the licensee's actions.
