- April 20, 1989 L PRELitilNARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PN0-IV-89-30 l This preliminary notification constitutes EARLY notice of events of POSSIBLE 4 safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the Region IV staff on this date.

SUBJECT:

AIT DISPATCHED TO RIVER BEND STATION At approximately 12 midnight April 14, 1989, a freeze seal failed on a 6-inch service water line. The freeze seal had been established to isolate two manual isolation valves to the auxiliary building unit cooler on the 141-foot level.

The valves were disassembled for repairs. The service water system was in operation at the time resulting in flooding portions of the auxiliary building 141,- 95 , and 65-foot levels before that portion of the service water system could be isolated. The water caused the loss of a transformer on the 95-foot level that resulted in the loss of the residual heat removal (RHR) system, normal spent fuel pool cooling, and normal lighting in the auxiliary bLilding and the reactor building. RHR was restored within approximately 10 minutes. Backup spent fuel pool cooling is available. The lost electrical systems will be repaired and returned.to service. All required safety systems are available in accordance with Technical Specifications. The licensee has the event under review. RBS is midway through a schedcled 50-day refueling outage.

The licensee has reported an 18 F temperature rise.over a 10-hour period in the spent fuel pool. The licensee plans to initiate backup cooling if the spent fuel pool temperature rises to 125 F. The safety analysis report indicates a temperature limit of 140*F. NRC Region IV is dispatching an Augmented Inspection Team (AIT) today to evaluate this event and GSU's overall management controis of outage activities. GSU has issued three stop work orders during the first 5 weeks of the outage related to the control of work activities.

Neither the licensee nor the NRC plans to issue a press release.

The state of Louisiana has been informed.

Region IV received notification of this occurrence by telephone from H00 at 6:40 a.m.on April 20, 1989. Region IV has informed EDO, NRR, and PA.
