MAJOR STEAM LEAK RESULTING IN PLANT SHUTDOWN (UPDATE)
The NRC resident inspectors have monitored licensee corrective actions relative to the gasket failure which resulted in a steam leak that led to a reactor scram ,
on July 8, 1986. The licensee has determined that inappropriate gasket material may have been used on the moisture seperator reheater drain tank manway cover and that improper torque was applied to the manway bolts following gasket replacement.on June 29, 1986. The licensee has replaced the "A" and "B" moisture separator reheater drain tank manway gaskets and they are investigating all high pressure 4
maintenance work, for the past two outages, for proper gasket and torquing applications. The licensee is formulating an action plan for corrective and preventive actions prior to restart of the unit and plans to issue a report to the NRC describing their findings and corrective actions. The resident inspectors will continue to monitor the licensee's actions.
