SUBJECT:

REACTOR TRIP AND HOMENTARY SAFETY INJECTION On March 1, 1986, the River Bend Station tripped from 38% power at 6:40 a.m.

(CST). The sequence of events leading to the trip was initiated by the failure of a nonvital electrical transformer. This resulted in a voltage transient which caused the one feed pump which was operating to trip. The operators started the Reactor Core Isolation Cooling (RCIC) pump, but before feed from the RCIC Dump was effectively established, the reactor tripped on low water level. The High Pressure Core Spray (HPCS) system also started, but it was secured in about 1 minute since reactor vessel water level was reestablished and was being controlled by RCIC. The licensee reported the trip and subsequently determined that an unusual event had occurred because of the HPCS injection. The reactor was restarted late on March 1. The senior resident inspector was contacted by the licensee at the time of the trip. The resident inspectors are monitoring licensee activities.

The state of Louisiana has been informed.
