Gulf States Utilities Company's (GSU) response to Notice of Violation 458/8724-02,'" Failure to Utilize Proper Revision of Surveillance Test Procedure," is provided in the enclosed attachment pursuant to 10CFR2.201. This completes GSU's response to this item.

ATTACHMENT Response To Notice of Violation 50-458/8724-02 Level IV

REFERENCE:

_ Notice of Violation - letter from L-. J. Callan to J. C. Deddens, dated October 15, 1987.-

VIOLATION:

' Failure to . Utilize Proper' Revision of Surveillance Test Procedure -

Technical Specification 6.8.1.g requires that written procedures be established, implemented and maintained for activities implementing the fire protection program.

Administrative' Procedure-(ADM)-0015, " Station Surveillance Test . Program,"

requires than an ' individual assigned to perform a surveillance test utilize anL official work copy of the. latest test , revision of the surveillance test procedure '(STP) and. any associated temporary change '

notices.

Contrary to the:above, STP-000-3001, " Daily Fire Door Position Check,"'

performed on September 20. 1987, and September 22, 1987, did not utilize-the latest revision of the.STP which was available from station document

" ,ntrol'on August 27, 1987.

. REASON FOR THE VIOLATION:

In ' August. 1987, Security Compliance personnel- picked up several work copies of STP-000-3001, " Daily Fire Door Position Check", from Plant  !

Document Control. On September 20 and .again on September' 22, the individuals failed to verify that they had the latest revision to STP-000-3001, Revision 2, dated August 27,.1987 and proceeded to conduct the surveillance tests using the out-of-date August 26, 1987.STP. Failure to follow procedure was a personnel error which has been isolated and corrected as described.

CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED:

On September 25, 1987, when the use of an incorrect procedure was identified, all work copies of STP-000-3001 maintained at the Security Compliance Section were immediately destroyed. Security Compliance personnel are required to obtain an official work copy of the STP from Plant Document Control on the day the surveillance test is to be performed with no exceptions.

Security personnel have been re-instructed, pursuant to ADM-0015, to verify ~with document control or the Station Operating Manual (SDM) Index maintained in the Control' Room that the documents to be used are current

' prior to performing the work.

A review was performed of STP documentation completed within the last six months and no similar occurrences were identified prior to August 27, 1987. GSU has determined'this use of an incorrect revision from August 27

- to September 24, 1987, to be an isolated case.

CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS:

In addition to steps taken as listed above, Plant Security has been placed on Document Control distribution list to receive copies of Temporary Change Notices and Procedure Changes. This along with above mentioned personnel training should prevent recurrence of the oversight.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED:

Compliance to Procedure STP-000-3001 has been fully achieved.
