a3 River Bend Station - Unit 1 Refer to: Region IV Docket No. 50-458/ Report 85-74 This supplemental letter responds to the Notice of Violation contained in NRC I&E Inspection Report No. 50-458/85-74. The 1 inspection was performed by Mr. W. R. Bennett during the period October 21-25, 1985 of activities authorized by NRC Operating License NPF-40 for Riser Bend Station Unit No. 1.

Gulf States Utilities Company's (GSU) supplemental response to the Notice of Violation 85-74-01, " Failure to Have Required Documents in Quality Data Packages", is provided in the enclosed attachment. This completes GSU's response to the Notice of Violation.

ATTACHMENT Response to Notice of Violation Severity Level IV 50-458/85-74 REFERENCE Notice of Violation -

REASON FOR VIOLATION During the October 21-25, 1985 inspection, Quality Data packages containing documents such as Certified Material Test Reports (CMTRs) which would include material property and heat treatment records for General Electric (GE) Master Parts List (MPL) item number C4t-F004A and B (serial numbers 464 and 465 respectively) could not be located on site.

Quality data package MPL No. C41-F004, Serial Nos. 464 and 465 for the standby liquid control system explosive valves did not contain material property records and heat treatment records.

GSU determined that GE returned these MPL items to the vendor for re furbishing . Valves with different serial numbers were returned to the site. Due to an oversite at GE, the required records were not submitted.

CORRECTIVE ACTIONS TAKEN AND RESULTS ACHIEVED Upon determining that the subject documents were not available, GE immediately telecopied contents of the data packages to the RBS site. Hard copies were subsequently sent via mail. GSU Quality Engineering personnel have verified these documents to be contained in the ASME N-5 Report Documentation File.

CORRECTIVE ACTION TO AVOID FURTHER VIOLATIONS A review of selected documentation was undertaken to ascertain if similar conditions existed with other Quality Data packages for GE MPL items.

All documentation which was shipped to the site directly from GE supplier's was reviewed. These direct shipped records were considered to be the most vulnerable to retrieval problems since they were not processed through GE in San Jose, CA as were other records. Of the ninety (90) directly shipped data packages, three (3) were not located on site but were available from GE.

Further investigation, via review of additional documentation related to one hundred (100) MPL items, did not reveal any deficiencies.

The results of the review of both document types indicate no additional corrective action is necessary.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance was achieved upon receipt of the documents. The final package was received on January 6, 1986.
