Refer.to Docket No. 50-458/85-49 Reason For Violation-Contrary _to -Administrative Procedure ADM-003, " Development,

. Control ?and Use of' Procedures," Rev. 6, fuel. transfer. operations were being conducted without the Temporary Change Notice (TCN) in use- stamped "TCN : SHIFT SUPERVI3OR-APPROVED COPY" in~ red ink as required. Furthermore, the TCN was not followed- exactly as written.

Corrective Steps Taken and ResultscAchieved Fuel transfer. ' operations were immediately halted and the procedural deficiencies were corrected.

CorrectiveMSteps Which Will Be Taken to Avoid'Further Violations

A-Memorandum was issued to' each- individual'
in the Operations Group of River Bend Station re-emphasizing the. requirement for.

procedural. compliance.

Corrective Steps Taken and Results Achieved FSAR paragraph 14..2 12.1.44.1.b describes the test objective of

the. ECCS/LOOPEPreoperational Test as demonstrating, in part,.the

! ability of the RHR, LPCS and HPCS systems to realign and inject rated -flow to..the' vessel-within the prescribed period of time.

1This paragraph-was understood by GSU to not require the RHR system to.' recover 'from secondary . modes of operation, such.as-

'22A3845, Paragraph 4.2.5.3.1.which states:

required to recover from. secondary modes of. operation, such
