UPPER GUIDE STRUCTURE LIFTING RIG FAILURE - UPDATE This PN updates PN0-85-105, issued on November 6.

Florida Power and Light (FPL) succeeded in lifting the core internals upper guide structure fre] of the reactor vessel about 5 p.m. (EST) on Saturday, November 9. The structure was placed in its planned location in the refueling cavity.

No problems were encountered during the lifting operation, carried out with a specially designed and fabricated secondary lifting rig. After the upper internals were successfully removed, a preliminary inspection revealed some minor damage to the regular lifting rig, but no significant damage to either the upper guide structure or the core barrel.

FPL also terminated the notification of unusual event, which was declared on November 6 wh:n one of three lifting rig attachments gave way, leaving the upper guide structure suspended in a tilted position with its bottom about two feet below the reactor vessel flange.

Region II is monitoring the review by FPL and Combustion Engineering, nuclear steam supply system vendor, of the circumstances of this occurrence.
