NOTICE OF VIOLATION Union Electric Company Docket No. 50-483 Operating License No. NPF-30 As a result of the inspection conducted on August 4-7, 1987, and in accordance I
with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
Callaway Technical. Specification 2.2.1 states, in part, that the reactor trip system instrumentation shall be set consistent with the trip setpoint values shown in Table 2.2-1.
The trip setpoint for overtemperature delta-temperature was specified in Note 1 of Table 2.2-1, which states, in part, that the overtemperature delta-temperature trip setpoint be automatically reduced by 1.91% of its value at design thermal power if delta-I (measured flux difference) is less than -35% and automatically reduced by 1.89% of its value at design thermal power if delta-I is greater than the +6%.
Note 2 of Table 2.2-1 states that the maximum trip setpoint for overtemperature delta-temperature shall not exceed the actual value by more than 2%.
TS 2.2.1.b states, in part, that with the reactor trip system instrumentation less conservative than the value shown in the allowable values column, either adjust the setpoint-or declare the channel inoperable and apply the applicable action statement requirement of TS 3.3.1 until the channel is restored to operable status with its setpoint adjusted consistent with the trip setpoint value.
TS 3.3.1 specifies a limiting condition of operation (LC0) allowing only one channel of the overtemperature delta-temperature trip system to be inoperable whenever the plant was operated in Modes 1 or 2.
TS 3.0.3 requires that the plant be in hot shutdown within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> whenever more than one channel is inoperable.
Contrary to the above, all four channels of the overtemperature delta-temperature trip setpoint were inoperable during the period November 1984 to June 25, 1987, in that the trip setpoint would not be reduced automatically unless delta-I was less than -70% or greater than +12%.
During this period, the plant was operated in Modes 1 and 2.
This is a Severity Level IV violation (Supplement I).
The inspection showed that action had been taken to correct the identified violation and to prevent recurrence.
Consequently, no reply to the violation l
