APPEhDIX A Namco Controls, Incorporated Docket No. 99900378/87-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on August 10-14, 1987 it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A. Section 11 of Namco Standard Practice Manual 20-0001 dated July 1985, Revision A states, "on no less than an annual basis, the director of engineering or his designee shall conduct a seminar for the review of the applicable portions of the Namco Controls QA Manual and Namco Standard practices.

Contrary to the above, Namco's last seminar was conducted on January 3, 1966.

B. Section 12.6 of Namco Standard Practice Manual 20-0008 dated July 1985, Revision A states, "All items calibrated will be identified by a

calibration" label showing date last calibrated, by whom, and when the next calibration is due."

Contrary to the above, the calibration label for thermocouple serial number TC-11 did not identify the date last calibrated and had the wrong date identified for when the next calibration was due. Also, the calibration due dates for the same instrument were recorded on the gage record card as one date and as a different date on the instrument equipment sheet of Test Report T.R. 308-1.
