l During an inspection conducted on August 24-28, 1987, the implementation of the )

Quality Assurance (QA) Program at the Combustion Engineering (CE) fuel manu- {

facturing facility in Windsor, Connecticut was reviewed with respect to the i fabrication of nuclear fuel assemblies. The applicable QA program requirements i I

are 10 CFR Part 50, Appendix B, 10 CFR Part 21, and CE's QA Manual (QAM), Revision dated May 4, 1987. Based on the results of this inspection, it appears that certain activities at the CE facility were not conducted in accordance with these commitments. These items are listed below.

1. Criterion VIII of Appendix B to 10 CFR 50 requires that identification of l an item be maintained on records traceable to the item throughout the l fabrication of the item.

Contrary to the above, Section 8.4.1.7 of the CE QAM, Revision dated l May 4, 1987, states, in part, that " Traceability when required by l contract, shall be maintained." This implies an exception if not required .

2. Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Contrary to the above, the CE QAM, Revision dated May 4, 1987, does not referenceTemporaryShopInstructions(TSIs)andtherearenodocuments available to describe how to use a TSI relative to the CE QA program.

3. Criterion V of Apperdix B to 10 CFR 50 requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Sections 3.2.2 and 4.0 of QC-15-14, " Procedure for Special Procesc Qualification," Revision 1, dated March 18, 1987, requires that MFG procedures inc'lude a list of critical parameters and the range of each and also procedures to evaluate parameters and criteria for results.

Contrary to the above MFG-15-05, " Process Specification for Grinding Pellets," Revision 0, dated Novomber 23, 1971 does not contain parameters or reference procedures. (87-01-03)

4. Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be accomplished in accordance with documented instructions, procedures, or drawings.
