l APPENDIX B NOTICE OF NONCONFORMANCE Weided Tube Company of America Docket No.: 99901176/90-01 Chicago, Illinois During an inspection conducted at the Welded Tube Company of America (WTC),

Chicago, Illinois facility on July 16-18, 1990, it was determined that certain activities were not conducted in accordance with the Nuclear Regulatory-Cossnission (NRC) requirements which are contractually imposed on WTC by.

l surchase orders with NRC licensees. These items are set forth below and have seenclassifiedasnonconformancestothe.requiremertsof10CFRPart50,(PO),

Appendix B, imposed on WTC by: (1) contract, (2) customer purchase orders and (3) the WTC Procedure & Quality Assurance Manual (PQAM), Revision No. 4

. dated May 1, 1986.

A.

Criterion II, " Quality Assurance Program " of Appendix B to 10 CFR Part 50, requires, in part, that indoctrination and training shall be provided to personnel perfonning activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.

Contrary to the above, WTC failed to establish in the PQAM or implement a program to provide for indoctrination and training of personnel performing i

activities affecting the quality of manufacturing and process inspection of safety-related structural steel tubing sections.

B.

Criterion IX, " Control of Special Processes," of Appendix B to 10 CFR Part 50,. requires, in part, that measures shall be established to assure that special processes, including welding, are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, staadards, specifications, criteria, and other special requirements.

Contrary to the above, WTC failed to establish in the PQAM or provide

. documented procedures to control activities affecting the quality of the

. electric resistance welding process used in the manufacture of safety-related structural steel tubing sections.

Criterion X, " Inspection," of Appendix B to 10 CFR Part 50, requires in l

part, that a program for inspection and process monitoring of activities affecting quality shall be established to verify conformance with the

documented instructions, procedures, and drawings.

L Contrary to the above, WTC failed to establish in the PQAM or implement a

program to control the inspection and process monitoring of the manufac-turing of safety-related structural steel tubing sections.
