APPENDIX B Kilbourn Engineering Company ,

Docket No. 99901107/88-01 NOTICE OF NONCONFORMANCE During the inspection conducted July 11-15, 1988, the implementation of the l QA program at ti,2 KE facility in Milwaukee, Wisconsin was reviewed with respect to the manufacture of fasteners for nuclear facilities. The applicable QA require-ments are 10 CFR Part 50, Appendix B, and KE's Quclity System Manual (ASM), dated '

September 18, 1986. Based on the results of this inspection, it appears that certain activities were not conducted in accordance with these requirements, The following nonconformances were identified:

1. Subsectin NCA-3867.4(a) of Section III of the ASME Code states, in part, "The Certified Material Test Report [CMTR] shall include the actual results of all required chemical analyses...."

Paragraph 3.5 of Soction 6.1 the QSM states, in part, "The CMTR shall include the actuel results of all chemical analysis...."

Contrary to the above, KE did not include the chemical product analysis which had been performed by Schevers Laboratories on CMTRs dated February 24, 1986 to Consumers Power and August 17, 1987 to Cleveland Electric Illumina-ting Company. (68-01-02) 1

2. Criterion XVII "Quality Assurance Records" of Appendix B to 10 CFR Part 50 requires that QA records such as qualifications of personnel be maintained. 1 Paragraph 3.1.5.1 of Section 2.1. of the QSM states, in part, "Personnel performing surveys shall be trained and qualified in the functions...."

Paragraph 3.1. of Section 11.1 of the QSM states, in part, "The following records shall be maintained by the Quality Assurance Department:

a. Personnel Qualification Records...."  ;

Contrary to the above, Kilbourn was unable to provide lead auditor c,ualification records for the Production Manager who erformed an audit of Schevers Laboratories on June 10, 1986. (88-01-03

3. Criterion IX "Control of Special Processes" of Appendix B to 10 CFR Part 50 requires that special processes including nondestructive testing be controlled.

Subsection T-170 of Section V of the ASME Code requires that the qualification of personnel be in accordance with SNT-TC-1A.

Section 9.6 of SNT-TC-1A (June 1980 Edition) states, in part, "... copies  !

of the employer's written practices shall be maintained on file...."

.. i Contrary to the above, Kilbourn did not have a copy of written practice No. 99-CNTP-002 dated April 15, 1982 for Conam Inspection who provides NDT Level III services for Kilbourn. (88-01-04)

4. Criterion VII, "Control of Purchased Material, Equipiaent, and Services,"

of Appendix B to 10 CFR Part 50 requires that the purchase of services be accomplished in a controlled manner which includes source evaluation.

Paragraph 3.3 of Section 3.1 of the QSM states, in part "These sources shall be approved and shall be placed on the Qualified Suppliers List

[QSL] prior to award of a purchase order...."

Contrary to the above, Kilbourn issued PO 8049 dated July 5,1983 to Starrett for calibration services without Starrett being on Kilborn's QSL.
