'Meredith Corporation Docket No. 99901129/89-01 APPENDIX A NOTICE OF NONCONFORMANCE During inspections conducted on November 2-4, 1988, ano February 1-3, 1989, the implementation of the Quality-Assurance (QA) program at the Meredith Corporation in Hillside, New Jersey was reviewed with respect to the supply of material to the nuclear industry. The applicable QA program requirements are-Appendix B to'10 CFR 50,10 CFR-Part 21,Section III of the ASME Boiler and Pressure Vessel Code, and various Meredith manuals and procedures.
Based on the results of the-inspection, it appears.that activities at'the Meredith facility were not conducted in accordance with these commitments. These items are listed below.

Subsection NCA 3867.4(e) of Section III of the ASME Code requires that in order to certify stock material as meeting the requirements of Section-III, a material supplier must perform a product analysis on each piece of stock material to verify chemical composition.
If the supplier cannot establish traceability for the material purchased, he must perform all other requirements of the material specification.
Contrary to the above Meredith certified and supplied stock material as meeting Section III to the Anchor Darling Valve Company,. South Texas nuclear plant, and Congdon and Carpenter without testing each piece of i
stock material. Meredith had not audited the quality' system programs at the suppliers and therefore could not establish traceability.-

. Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.

Revision 0, dated February 20, 1988, requires that nuclear sales order folders be color coded for the control of documents on nuclear procurement.
