This letter addresses the inspection of your facility at Mentor, Ohio conducted l by Mr. R. N. Moist of this office on August 10-14, 1987, and the discussions of j his findings with Mr. D. A. Coe and other members of your staff at the conclu-sion of the inspection.
The purpose of the inspection was to perform a technical evaluation of your recent equipment qualification test activities for safety-related equipment and to verify continued implementation of your Quality Assurance Program. Areas examined during the NRC inspection and our findings are discussed in the enclosed report. This inspection consisted of an examination of procedures and representative records, j
interviews with personnel, and observations by the inspector.
i During this inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Specifically, instances were noted where ,
personnel failed to satisfy the requirements of Namco's Standard Practice Manual, to !
conduct indoctrination training seminars on a yearly basis and identify information '
correctly on calibration labels. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please provide us within 30 days from the date of this letter a written statement containing: (1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventative measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
The responses requested by this letter are not subject to the clearance proce-dures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
