NOV o 3 is Babcock & Wilcox Fuel Company Docket No. 99900001/90-01 APPENDlX A NOTICE OF NONCONF0Pf4ANCE During an inspection conducted on October 15-19, 1990, the iniplementation of 4
the Quality Assurance (QA) program at the Babcock & Wilcox Fuel Company's Cormercial Nuclear Fuel Plant (CNFP) in Lynchburg, Virginia was reviewed with respect to fabrication of fuel assemblies supplied to the nuclear industry.
The applicabic QA program requirements are Aspendix B to 10 CFR Part 50,10 CFR Part 21. Section !!! of the ASMF Boiler and )ressure Vessel Code and various CNFP manuals and procedures.
Based on the results of the inspection, it b
appears that activities at the CNFP facility were not conducted in accordance with these commitments, These items are listed below.
1.
Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality be prescribed by documented procedures and be accom-plished in accordance with these procedures.
Section 5 of QC-506, ' Receiving Inspection-Lower End Fitting Mark C.'
Revision 1, dated October 27 1980, requires that dimensions of lower end fittingsbem:asuredanddevIationsreportedtotheInspectionSupervisor end nonconforming conditions processed accordingly.
Contrary to the above, CNFP failed to identify excessive raised metal in the form of a burr adjacent to one grid sad bore hole on the KMC supplied lower end fitting marked with serial num>er 15273-04.

CNFP procedure QC-1414. " Retention and Storage of Quality Assurance Records', Revision 4, dated September 13, 1990, is the procedure that documents activities for QA records.
Contrary to the above, QC-1414. Revision 4, does not provide requirements for storage and retention of X-ray film (radiographs), and does not identify whether the radiographic film and the radiograph reviewer reader i
sheets are considereo quality records.
