APPENDIX A Bechtel Eastern Power Company Docket No. 99900519/88-01

NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on April 11-13, 1988, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states:
"Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Bechtel Engineering Department Procedure 4.38, Revision 1, dated November 30, 1984, "Computer Program Error Reporting and Corrective Action," states in Section 5.2 that computer code program sponsors are responsible for obtaining sufficient information on program errors to classify such errors and to describe how the error may have affected the program results on a generic basis, in addition Section 6.0, "Notification to Users and Others," specifically Section 6.1, states that computer code program sponsors are required to initiate the error reporting process which promptly advises Bechtel users registered with Bechtel's Central Information Services Library and appropriate chief engineers of errors.
Contrary to the above, the Bechtel code sponsor, for the GT STRUDL computer program, failed to perform such actions on error notices sent him by the Control Data Corporation from the period January 1985 to the present (approximate).
