APPENDIX A Westinghouse Electric Corporation Inspection Report Nc. 99900404/88-01 -

NOTICE OF N0NCONFORMANCE Based on the results of an NRC inspection conducted on April 18-22,1988, i t appears that one of your activities was not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished ir, accordance with these instructions, procedures, or drawings. Instructions ,

procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Westinghouse operating procedure WRD-0PR-19.0, Revision 2, "Identification and Reporting of Substantial Safety Hazards, Significant Deficiencies, and Unreviewed Safety Questions," dated December 1,1985, specifically Appendix A Section E, "Time Limits for Evaluation of Potential Substantial Safety Hazards and Unreviewed Safety Hazards," states that items potentially affecting the safety of any operating or near term operating plant should be evaluated as quickly as the nature of the itet; permits.

Contrary to the above Westinghouse failed to evaluate and report to the NRC,  !

in a timely manner, a correction to the original Part 21 reoc-t, dated July 13, 1984, despite informotion known to Westinghouse, as early as April 1985, l recognizing that implementation of the original recommendations may lead to a i possible violation of containnent isolation criteria for certain plants. until approximately two years later. This issue related to a potential over-pressure condition in the conponent cooling water systems designed by )
