Sorrento Electronics Docket No. 99900387/87-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 16-20,.1987 it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or orawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or. drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
QA Procedure No. 9-01, " Verification of Special Process Controls," states in part, "when supplier audits are performed per contract to assure control of special processes, the auditor shall verify and document that qualified personnel, equipment and procedures are used..."
Contrary. to the above, a review of sub-tier vendor's welder qualification /
procedure records from Lemke Manufacturing and Aerated Engineering determined that three out of the four records that were reviewed by SE did not indicate that SE had performed its required verification to assure that qualified personnel and procedures are used in the vendor's qualityactivities(87-01-02).
B.
Section 5 of the SE QA manual, " Instructions, Procedures, and Drawings",
states, in.part:
"Sorrento Electronics' Managers and Supervisors shall ensure that for activities affecting quality, written instructions, procedures, or drawings are prepared, reviewed and approved... quantita-tive criteria, such as dimensions, tolerances, and operating limits,...
shall be specified, as appropriate, for determining satisfactory work performance and quality compliance. These quantitative or qualitative criteria must be suitable for inspections, tests, or audits...."
i Contrary to the above, SE failed to provide approved instructions to its assembly personnel for cable end preparations for Belden coaxial cable type 9266, and its associated MHV coaxial cable electrical connector installations. The specific product work activities observed for this example involved the fabrication of the RD53 liquid monitor subassembly
-detectors that SE is manufacturing for several nuclear facilities.
