' APPENDIX A' NOTICE OF: VIOLATION ,

As a result of th'e inspection conducted during the period of~12/20/86 through' 2/7/87 a violation of NRC requirements was. identified. In accordance with the

-General Statement of. Policy and Procedure for NRC Enfo'rcement Actions, 10 CFR-Part 2 Appendix-C, as revised, 49 FR 8583 (March 8, 1984), the' violation is set' forth below:

A. Paragraph 6.8.1 o[ the, Unit 3 Technical Specifications states in part that:

6.8.1 Written' procedures shall be established, implemented and maintained covering the activities referenced below:

Fire Protection Procedurc 50123-XIII12 titled, " Control of Ignition-Sources," defines the following requirements:

6.2.1 A Flame Permit (Attachment 2) shall-be completed by-the Cognizant Supervisor or;his designee (see Definitions Attachment 1) prior to commencing any wo~rk involving an' ignition ~

. source.in the Protected Area.

6.2.2- -A Flame Permit is. valid for no more than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

If work cannot be completed within~24 hours,'a new permit shall be obtained.

6.5.4 Post'the issued Flame Permit on the fire extinguisher.

6.5.9 Keep the fire extinguisher within arms reach at all -

times.

Contrary to sthe above requirements, on January 20, 1987, the following condit' ions associated with DCP 3-6238.0N work activities on Unit 3 were observed: ,

1. Grinding work, requiring a fire watch, was in progress at 0930, although the flame permit covering this work had expired at 0600.
2. The fl'ame permit was not posted on the fire extinguisher.

3.- Grinding continueil after the 7 fire watch left the area.

4. The fire extinguisher was not kept within arms reach of the fire watch.

This is a Severity Level V. Violation (Supplement I) applicable to Unit 3.
